Person who participate in the identification of the needs of the population participate in the community organization & development and execute different action programs by using appropriate techniques, may act as a resource person for the groups.
The incumbent participates in identifying the needs of the population served by the Kapataakan Waswanipi Pilot Project for the Community-Based Addiction/Wellness Recovery Program. He/she contributes to service delivery and project development, executes various tasks using appropriate techniques, and may act as a resource person for community partners.
The mandate is to provide continuous first-line services to individuals, youth ages 12 to 17, and adults 18 and older experiencing problematic substance misuse, as well as to their significant relationships. Meet and intervene with individuals, families, and the community in accordance with the pilot project framework, while respecting CBHSSJB policies and procedures.
• Assess the needs of those served, provide assistance, and refer them to appropriate programs within CMC resources or community partners when necessary.
• Collaborate with therapeutic colleagues and clients to define, review, and update personalized care plans, ensuring aftercare plans are co-developed in line with client goals.
• Ensure proper use of intervention and data collection tools.
• Carry out prevention activities, such as presentations, in collaboration with community resources when requested.
• Organize and facilitate information sessions and community activities.
• Contribute to pilot project evaluation activities.
• Perform any other duties related to the Community-Based Addiction/Wellness Recovery Program as requested by the supervisor.
Must have a diploma of college studies in social work techniques or social service techniques or another appropriate specialization from a school recognized by the competent ministry;
• This job title also includes those who have a certificate of college studies in social work techniques or social service techniques.
Work experiences in helping relationships and/or counselling will be an asset.
Solid knowledge of the Waswanipi social environment and community resources.
• Familiarity with Cree culture and social/health issues in Eeyou Istchee.
• Ability to write reports in English.
• Capacity to collaborate with CMC community workers, NNADAP workers, medical, psychosocial, and mental health staff, as well as other resources within Waswanipi’s community network.
• Willing to participate in training related to the project.
