Detailed Overview

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:

The Program Coordinator, South Asian Health Institute (SAHI), works collaboratively with Population and Public Health program managers and teams, community partners, and other FH program leadership in the identification of South Asian service needs, development of community engagement and the coordination, development and effective delivery of SAHI services.

Identifies health service needs and future demands related to the South Asian population: designs, develops and delivers education to meet the identified needs; promotes access to services by teaching, conducting presentations and facilitating groups in a variety of community forums and locations. Evaluates outcomes of the educational/training sessions; monitors service implementation in coordination and collaboration with other Programs. Attends meetings on behalf of the SAHI Team as requested to offer support, guidance and to identify partnership opportunities and resources; provides input for the development and maintenance of resources to support professionals and families regarding South Asian health.


Responsibilities

  1. Works collaboratively with Population and Public Health program Managers and teams, community partners, and other FH program leadership in the coordination of programs.
  2. Participates in the development and coordinates the implementation of program services. Develops, implements, evaluates and updates service/program specific plans, goals and objectives, policies and procedures, and related guidelines and documentation. Monitors implementation progress; initiates corrective action to ensure that program implementation strategies and services operate efficiently and effectively. Monitors service needs and future demands for SAHI.
  3. Works collaboratively with others to evaluate, identify and determine needs of the South Asian population based on assessment data; prioritizes identified needs and designs educational materials to meet those needs and promote access to services by teaching, facilitating groups and conducting presentations, demonstrations and lectures; evaluates short term and long term outcomes of the educational/training sessions. Regularly assesses progress toward related goals and outcomes.
  4. Coordinates and liaises with appropriate stakeholders (internal and external) and community/cultural groups to collaborate on the effective delivery of SAHI services; ensures coordination of information; facilitates program planning and offsite education sessions.
  5. Coordinates and provides support activities by organizing workflow, determining priorities, resolving day-today problems and evaluating operating procedures.
  6. Attends meetings on behalf of the SAHI Team as requested to offer support, guidance and to identify partnership opportunities and resources.
  7. Plans and attends regular team meetings; participates in relevant and related hospital and community based committees, activities and projects related to SAHI services; provides input for the development and maintenance of resources designed to support professionals and families regarding South Asian health.
  8. Discusses inquiries for information with a variety of internal and external contacts; answers routine inquiries by determining the nature of the issues and provides information directly or through correspondence.
  9. Supports training, supervision, and/or evaluation of wellness ambassadors, as assigned.
  10. Performs other related duties as assigned.

Qualifications

Education and Experience

Bachelor's Degree in a health science, social science or related discipline with coursework/studies in program evaluation, plus three (3) years' related experience in public health, community health programs, acute care or other health services, including experience in program planning and/or program evaluation; or an equivalent combination of education, training and experience.

Proficiency in either Punjabi, Hindi, Urdu or other South Asian Language, is an asset.

Valid BC Driver’s License; access to a personal vehicle for business purposes.

  • Physical ability to perform the duties of the position.
