Your Opportunity:

Family health teams provide comprehensive healthcare for individuals and families. The role of the physiotherapist in the Hamilton Family Health Team (HFHT) is to complement existing services and improve the care of complex patients including those experiencing chronic pain and other chronic health conditions.

The physiotherapy (PT) role will focus on high need populations who do not otherwise have access to physiotherapy supports in the community. The successful candidate will be responsible to assess and treat individuals who self-refer or are referred by HFHT physicians. Additional post-graduate training in pelvic health physiotherapy is preferred.

The PT will diagnose within their scope of practice and establish and implement evidence informed treatment plans that include integration with existing community services. The PT will participate in and lead group programs within the HFHT, such as a chronic pain self-management group program and osteoarthritis group exercise programs. Expertise in the GLAD program would be an asset.

A strong knowledge of community-based programs and services to facilitate clients’ access to external services to meet individual needs and manage caseload demands.

Requirements and Responsibilities:

  • Successful completion of an entry level BSc (PT), MSc (PT) or equivalent
  • Current registration in good standing with the College of Physiotherapists of Ontario
  • Primary Care experience is preferred along with a supportive approach to patients that may struggle with social determinants of health.
  • Pelvic health physiotherapy post-graduate training is preferred
  • Proficiency completing clinical assessments and evidence-based care plans for patients with complex conditions, including chronic pain while working with them to develop self-management skills.
  • Experience working as part of an inter-professional team
  • Ability to build effective, professional relationships with patients, provide a supportive approach to care and guide patients to work towards independence.
  • Strong communication and documentation skills
  • Understanding of quality improvement processes as they relate to patient care and program evaluation.
  • Creating and delivering population health interventions that include all levels of prevention (i.E., primary, secondary, tertiary)
  • Experience working with computers and technology (i.E. EMRs, Microsoft Teams)
  • Experience facilitating group education sessions is preferred
  • Experience or interest in supervising student learners from physiotherapy and other health care professions
  • Knowledge of Hamilton health and social services
