POSITION SUMMARY
The Nurse Specializedin Wound, Ostomy and Continence is responsible for providing support, treatmentrecommendations and follow-up care on identified problem wounds andostomies.Functions of the role mayinclude consultation, direct patient care, education and research.The Nurse Specialized in Wound, Ostomy andContinence facilitates the utilization of the wound care program by assistingwith staff and service provider education as well as to Home and Community Carepatients upon referral from Care Coordinators and in accordance withprofessional college standards, Ministry guidelines and Home and Community CareSupport Services policies.
QUALIFICATIONS:
- RegisteredNurse in good standing with the College of Nurses of Ontario (CNO);
- Certificatefrom Nurses Specialized Wound, Ostomy, & Continence Canada (NSWOCC) inWound, Ostomy and Continence required;
- In-depthknowledge of home health care and other community resources;
- Goodunderstanding of the roles of other health care professionals;
- Knowledgeof funding agencies available to support patients;
- Demonstratedcase management, assessment and interviewing skills;
- Practicalknowledge and understanding of relevant legislation (e.g., regarding theprovision of health care services, privacy, health, and safety, etc.);
- Comprehensiveknowledge of the standards of practice and professional guidelines setforth by the College of Nurses of Ontario and Nurses Specialized in Wound,Ostomy, & Continence Canada (NSWOCC);
- Abilityto prioritize professional duties, manage multiple patients, andefficiently organize workload;
- Stronginterpersonal and collaboration skills to work with diverse patientgroups, care coordinators, family members/caregivers/Substitute DecisionMakers, health care professionals, community organizations and serviceproviders;
- Effectivelistening, observation, and facilitation skills;
- Abilityto communicate information effectively through a variety of meansincluding reports, letters, meetings, and presentation;
- Workingknowledge of computer software (email, internet) and Microsoft Office applications(Word, Excel);
- Must have valid driver’s license and access to avehicle;
- Valid Emergency First Aid Certification;
- Oraland written proficiency in English is essential.
