Bayshore HealthCare has been a recipientof Canada's 50 Best Managed Companies award since 2006.Job SummaryThe Nurse Case Manager will be crucial in evaluating medical fitness to work while ensuring compliance with legal, regulatory and policy requirements. As an integral member of a multidisciplinary team, the Nurse Case Manager’s duties include assessing disability and medical fitness for duty of individuals occupying safety sensitive or safety critical positions, non-safety sensitive positions with physical demands as well as for other positions. Additionally, the Nurse Case Manager is always responsible for collecting and safeguarding highly confidential medical information about applicants, employees, and former employees.Duties and Responsibilities-Work autonomously, remain focused in a fast-paced environment, and execute sound clinical judgment.- Assess and monitor fitness to work of applicants and employees using specific indicators and approved criteria, and in accordance with policies, practices, and procedures.-Perform medical evaluations for pre-placement and monitor cases as required.-Assess substance use in accordance with the Drug and Alcohol Policy.-Learn and apply electronic documentation systems, and other required technology associated with job activities.-Collect medical data objectively and record and interpret it concisely and accurately.-Keep all personal health information confidential as required by standards of practice and legislation, including that which is documented or stored electronically.-Maintain complete and accurate employee health records (electronic and paper files) respecting legislative requirements, security, privacy and confidentiality.-Maintain competency in areas such as critical thinking, the use of evidence-based information, communication, and interpersonal skills.Job QualificationExperience-Minimum 3 years experience in disability management or in acute nursing care.-Excellent knowledge of disability management (occupational or non-occupational).-Excellent telephone communication skills.-Excellent attendance record.Education-Current Registered Nursing licensure in good standing in the province of Ontario.Other Information-Remote work area (home office); all equipment and necessary resources will be provided.Carepath is an Equal Opportunity Employer.
