Mint Health + Drugs is an established independent pharmacy committed to delivering outstanding patient-centred care and specialized services. This hybrid role involves a combination of responsibilities in accordance with provincial and federal laws, standards, and regulations governing the practice of pharmacy, the Ministerial Order for Clinical Pharmacy Services, site policies, and internal Mint Health + Drugs policies and procedures.
Dispensary Fulfillment Operations:
The pharmacist will manage the execution and distribution of medications for contracted homes, detox facilities, emergency shelters, and other programs we service.
Triaging and processing all medication orders for the pharmacy.
• Overseeing pharmacy operations to ensure efficiency and optimal patient care.
• Accurately and promptly packaging and dispensing prescriptions.
• Training and mentoring pharmacy team members as needed.
Clinical Outreach:
The pharmacist will ensure continuity of care for clients through collaboration with onsite physicians, nursing staff, and other health providers at long-term care, supportive living, and other congregate living communities.
Providing clinical pharmacy services to patients in alignment with Continuing Care Service Standards, site policies, internal policies, and the Ministerial Order of Clinical Pharmacy Services.
• Conducting annual and quarterly medication reviews, hospital discharge medication reconciliations, etc.
• Establishing collaborative practice networks with physicians, nursing staff, health care aides, case managers, and other facility staff.
• Adhering to internal policies and processes established by the Clinical Division.
• Supporting operational activities through coordinated service delivery, facilitating site-to-pharmacy communications, and ensuring continuity of care for patients.
• Bachelor’s degree in pharmacy and valid licensure.
• Proficiency in pharmacy software and database programs.
• Remains current with advances in health and pharmacy-related matters.
