We are searching for a Permanent Full Time Resident and Family Services Coordinator  to join our  Colchester/Cumberland  team based in Bible Hill, Nova Scotia.

About the Opportunity

Prepare reports, records, statistics and other data as needed or requested. Participates as member of the interdisciplinary team in team, client and family meetings. Collaborates with clients and family/friends to meet their program expectations and needs. Conduct tours of the facilities and interviews prospective clients and/or families through home, hospital and/or office visits to obtain pre-admission information. Coordinates admission, transfer and discharge of clients in coordination with the interdisciplinary team. Prepares funding requests and reports relating to client needs; maintains client record as required. Promotes safety culture (or act as a safety role model) by taking every reasonable precaution in the circumstances to ensure the health and safety of employees and all others in the workplace. Ensures client safety by following and applying all standard operational procedures.

About You

Currently a Licenced Practical Nurse or Registered Nurse actively registered with Nova Scotia College of Nursing CPR certified Previous experience working in long term care or with seniors. Knowledge of the principles of human behaviour and ability to apply program solving skills. Strong computer literacy skills. Ability to communicate effectively and efficiently (both verbally and in writing). Knowledge of available community program and supports. Ability to establish and maintain effective, cooperative working relationships with staff, families, clients and other stakeholders.
