About the OpportunityReporting to the Health Services Manager, the Health Care Social Worker is a member of a highly collaborative interprofessional team, and plays an important role in the delivery of Primary Health Care, chronic disease management care, with a focus on chronic pain. Social Workers are primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problem solving skills, past successes, and personal resources.
About YouWe would love to hear from you if you have the following:
- Master's degree in social work from a recognized university
- Current registration with the Nova Scotia College of Social Workers
- 2 years post MSW related clinical experience or relevant work experience required
- Previous experience working in Primary Care and/or community-based setting preferred
- Demonstrated ability to use a client-/family-centered approach
- Psychotherapy skills (i.e. ACT and CBT)
- Familiarity with various evidence-based treatment approaches including Motivational Interviewing and behaviour change counselling
- Demonstrated experience with group facilitation
- Ability to complete biopsychosocial assessments
- Excellent communication skills
- Demonstrated ability to assist patients and families with navigation of local community resources and agencies
- Ability to demonstrate effective time management and disciplined self-organization
- Demonstrated comfort and skills working collaboratively with an interdisciplinary team and community partners
- Demonstrated ability to communicate effectively with patients, families, public, community partners, primary care team members and other professionals and agencies using verbal, written and computer communication means
- Demonstrated ability to work both independently and collaboratively within an interdisciplinary team setting
- Demonstrated ability and comfort in working flexibly and creatively to provide services
- Demonstrates commitment to diversity, equity and inclusion
- Awareness of local community resources and agencies would be considered an asset
- Valid Nova Scotia Driver's license and access to a reliable vehicle
- Competencies in other languages preferred; French an asset
- Competencies in other languages an asset, French and Mi'kmaw preferred

Hours- Permanent full-time position; 75 hours bi-weekly
- Hours include days and may include evenings and weekends.
- Travel to various sites may be required
$37.31 - $49.04 Hourly
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
