Senior Full Stack Developer – Remote Canada Join to apply for the Senior Full Stack Developer – Remote Canada role at Smile Digital Health. The Senior Full-Stack Developer will have deep experience in FHIR-based interoperability to support the delivery of large-scale healthcare data exchange solutions supporting CMS mandates such as Patient Access, Provider Access, and Prior Authorization. This role is responsible for writing production-grade code, leading technical delivery, mentoring developers, and collaborating closely with Project Managers, Solution Architects, and Technical Account Managers to ensure that every deployment meets both performance and compliance standards. Responsibilities Develop backend services, adapters, and integrations using Java, Spring Boot, and Apache Camel for CMS mandates (Patient Access, Provider Access, P2P, Prior Auth) Deliver FHIR R4+/SMART on FHIR APIs aligned with US Core and applicable Implementation Guides, ensuring full terminology compliance (LOINC, SNOMED CT, RxNorm, ValueSets) Participate in customer meetings, office hours, demos, and escalations; explain technical concepts clearly and professionally Code & Test Excellence – Write clean, maintainable code with strong unit/integration testing (JUnit, Testcontainers, MockMVC) and participate in detailed peer reviews Automate & Deploy – Build CI/CD pipelines in GitLab, manage Helm deployments on Kubernetes, and troubleshoot services, pods, and performance issues Integrate & Scale – Work with Kafka or equivalent messaging queues and relational databases (PostgreSQL/MS SQL) to deliver scalable, event-driven data pipelines Mentor & Lead – Review code, guide junior developers, and promote best practices in design, automation, and maintainability Accountable for ensuring that all working hours are accurately reported in the time tracking system on a daily or weekly basis, that the majority of (if not all) hours are tracked as billable and that the integrated project management tool is properly and fully utilized. Tracking and reporting of billable hours is a critical aspect of project management and delivery to our customers and this is a major area of accountability Requirements 8+ years software development experience, including 5+ years in Java/Spring Boot Proven delivery of FHIR/SMART on FHIR integrations in production environments Strong knowledge of US Core, FHIR IGs, and healthcare terminologies (LOINC, SNOMED CT, RxNorm, ValueSets) Prior experience in professional services or healthcare delivery teams Hands‑on with GitLab CI/CD, Kubernetes/Helm, Kafka, and relational databases (PostgreSQL/MS SQL) Excellent verbal and written communication and ability to work directly with customers Highly organized, accountable, and capable of managing multiple concurrent priorities Preferred Experience Experience with Smile CDR or HAPI FHIR Exposure to HL7 v2, CDA, or legacy FHIR integrations Familiarity with CMS interoperability mandates (Patient Access, P2P, Provider Access, PA) Experience authoring FHIR Implementation Guides or using Inferno for testing Familiarity with AWS/Azure cloud environments and infrastructure-as-code (Terraform) We may use artificial intelligence (AI) tools to support parts of the hiring process, such as reviewing applications, analyzing resumes, or assessing responses.
