Responsibilities Participate as a full and active member in the delivery of team-based primary care services, within a model that includes physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support. Provide timely access to patients, families and caregivers to comprehensive, person-centred, culturally safe and quality care. Contribute to extending intentional and coordinated care to all community members, including the medically complex and vulnerable. Flexible scheduling for 4- and 8-hour longitudinal or urgent care shifts.
