The Collaborative Practice & Education Specialist is a change leader, building capacity for clinicians to lead and translate evidence to practices, as well as implementing and evaluating complex practice change processes across the sites of Unity Health in alignment with strategic priorities. The Collaborative Practice & Education Specialist reports to the Manager, Clinical Practice Improvement & Scholarship. As a member of the Clinical Practice Improvement team, the Collaborative Practice & Education Specialist will support clinicians across the three sites of Unity Health Toronto through the implementation of practice changes to advance collaborative practice at the point of care, ensuring the highest quality, patient and family‑centered care through education, mentorship and change management strategies. Duties & Responsibilities Implement innovative and evidence‑based practices within clinical programs with the goals of: advancing interprofessional collaborative practice resulting in improved patient outcomes; and improving patient and staff safety through implementation of best practices in interprofessional practice. Collaborate with partners in Business Intelligence, Patient Safety, Practice Informatics, Operational Excellence, Quality, and Professional Practice to gather, analyze and share data and best evidence to define outcomes and process measures in the design and evaluation of strategic goals. Lead implementation of strategic practice change initiatives across Unity Health using a collaborative leadership style and human‑centered design principles. Lead evaluation of Professional Practice initiatives and prepare presentations and reports on outcomes and evaluation for purposes of scaling initiatives, reporting on Quality Improvement initiatives corporately and/or contributing to practice‑based research. Work collaboratively with the Clinical Practice Improvement Team to support and facilitate key professional development programs such as the Health Disciplines Emerging Leaders Fellowship and Nurse Champions Program. Support the applications to grants and research funding opportunities for Professional Practice to continue to design and implement innovative and evidence‑based practice changes within clinical teams. Participate in formal and informal evaluation of Professional Practice programs by analyzing and evaluating data. Collaborate in the design of appropriate quality indicators for key Professional Practice initiatives and make recommendations regarding process improvement. Create, foster an environment and/or relationship with front line staff that encourages a continuous learning environment. Support practice‑based research, program evaluation, and quality improvement initiatives amongst professions. Serve as the Professional Practice lead for the GEMINI‑Delirium project, including overseeing the project locally, contributing to the design, data collection, and leading the implementation of the intervention on the chosen wards. Qualifications Must be a health discipline or nurse currently registered and in good standing with a regulatory college (e.g. College of Nurses of Ontario, College of Physiotherapy of Ontario). Must have a Master’s degree. Must have 5‑7 years of clinical experience as a nurse or health disciplines clinician. Demonstrated experience in data and measurement, including the ability to collect, analyze, interpret, and transform data (using tools such as Microsoft Excel), and effectively communicate results visually. 1‑3 years of healthcare leadership experience preferred; must be able to demonstrate experience leading projects, change initiatives, or teams. Current BLS (or willingness to complete within probationary period). Excellent communication, critical thinking, and interpersonal skills. Proven flexibility and adaptability in a fast‑paced environment. Excellent time management, organizational, and prioritization skills.
