LPN - Casual - Academy of Aging
The Academy of Aging is looking for a Casual LPN to join their amazing team.
***Must be Flexible in availability***
The LPN assists the Clinical Lead in the successful implementation of Resident focused goals and objectives for better outcome of each Resident's lifestyle and wellness needs. Duties include, but are not limited to, the direct supervision of Resident care positions (Personal Care Assistants), performing Resident assessments and management as directed by the Clinical Lead. The LPN ensures that Resident focused wellness needs are achieved through continual assessment and evaluation and quality improvement initiatives. The LPN is expected to carry out all roles and responsibilities in a manner that is consistent with the Core Values of the Company and must be ready and willing to respond to the emergency needs of all Residents in a timely and professional manner.
Requirements and Accountabilities
Resident Lifestyle Services
· Implement and administer personalized Wellness Care Plans for Residents
· Timely response to all emergency calls
· Ensure that a personal visit occurs following an incident, accident, injury or return from hospital
· Respond and report Resident/family requests/concerns to Clinical Lead
· Respond and report Resident's change of condition and/or needs
· Balance service demands with supporting Resident independence
· Identify, assess, report and document the clinical status of Residents
· Develop a personalized wellness plan (in collaboration with the Clinical Lead), based on the identified physical and psychological needs of the Resident including: the identified needs; Resident specific goals and planned program and/or interventions
· Coordinate wellness needs with Community providers via effective communication and team involvement process
· Measure and accurately record vital signs (temperatures, pulse, respiration, blood pressure, weight and height) where appropriate and as per practice guidelines
· Observe and report any significant changes in Resident behavior and health to the Clinical Lead, physician, family, and Director of Care
· Ensure that information obtained by the Wellness team is available to relevant members of the health care team, Resident and family consistent with confidentiality guidelines
· Arrange tray service for illness or as needed and check on Resident
· Perform wound management and injections
· Ensure Director of Care, Clinical Lead and family notified after an emergency or incident
Analysis and Reporting
· Assist in MAR and other audits as directed by the Clinical Lead
· Conduct quarterly chart and wellness plan reviews
· Actively participates in the QA process per assignment
· Participate/facilitate in staff meetings as needed or designated by Clinical Lead
· Participate in required quality improvement initiatives
· Maintain shift updates in communication log
Team Leadership
· Supervise all activities of the Resident care staff
· Serve as mentor to new wellness employees (PCAs.)
· Maintain duties as per shift roster/daily work schedules
· Assume leadership responsibilities as directed by the Clinical Lead in the implementation of Resident wellness programs (e.g. Yardi)
· Provide ongoing supervision of Residents' health related activities; comply with health care instructions explained by Clinical Lead; seek and obtain medical attention for Resident when needed.
· Ensure daily PCA assignments are maintained throughout each shift
· Participate in the orientation and performance appraisal process of PCAs, as required
Compliance and Safety
· Demonstrate infection prevention and control
· Use proper lifting techniques
· Ensure that narcotic records are accurate at the end of each shift
· Demonstrate working knowledge of standards and regulations regarding hazardous materials (e.g., chemicals, personal care products, and items locked)
· Identify safety hazards, work with wellness team in problem resolution and report same to the Clinical Lead
· Demonstrate use of personal protective equipment
· Assist Clinical Lead ensure compliance with medication management per regional regulations
Activities
· Oversee and/or conduct activities, where necessary
Facilities Maintenance
· Ensure that any in-use equipment is not left unattended
· Report any malfunctioning equipment to maintenance and Director of Care
Qualifications
· Current LPN License in good standing with the regional licensing body
· Proficiency in:
o MS office (Word, Excel, Outlook)
o YARDI EHR
· Understanding of advanced directives and end of life issues
· Understanding of the philosophy and LPNs' roles and responsibilities in the different levels of care
· Must possess current CPR "C"/AED certification and Standard First Aid Certification
· Understanding of pharmacological principles and the nursing process in the safe administration of medication to Residents, in keeping with the policies and procedures of MVLC
· Working knowledge of regional and federal regulations in seniors' health and housing
Physical and Sensory Requirements (with or without the aid of mechanical devices)
· Able to walk and possibly climb stairs
· Must be able to walk and stand for multiple hours in duration
· Ability to stand, walk, stoop, kneel, or crouch for prolonged periods
· The ability to turn, stoop, bend, stretch, and lift to assist residents in meeting their wellness needs and other related duties
· Use of hands to finger, handle or feel objects, tools or controls, with each hand/arm
· Medium Physical Demand Level: ability to regularly lift and/or move up to 9 kg / 20 pounds, and occasionally lift up to 23 kg/50 pounds
· Good visual acuity
· Good hearing acuity
· Mental acumen
Working Conditions
· Must possess a high degree of flexibility and tolerance to adjust to changes in workloads and days (i.e. weekends, evenings etc.)
· Prolonged standing and walking
· Lifting and assisting in transferring of Residents
· Lifting equipment and supplies
· Repeated bending and stooping
· Must be able to prioritize tasks and following procedures to meet the wellness needs of each Resident
