This position will operate as part of the new HART Hub. The HART Hub is an initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT) led by South Riverdale Community Health Centre and St. Michael's Homes with support from Anishnawbe Health Toronto, Gerstein Crisis Centre, The Neighbourhood Group, Michael Garron Hospital, Alpha House, Fontbonne Ministries, The Salvation Army, 2-Spirited People of the 1st Nations, Woodgreen Community Services, Comprehensive Treatment Clinic and The Neighbourhood Organization.

Providing primary health care, the Nurse Practitioner works collaboratively with other interdisciplinary team members, health care professionals/service providers, partners and service users. This position is required to work flexible hours, including evenings and weekends onsite, at partner locations, or through mobile work.

Responsibilities:

  • Develops rapport with service users and maintains therapeutic relationships within professional boundaries
  • Conducts comprehensive health assessments including a holistic health history, physical examination and appropriate screening and diagnostic tests
  • Assesses and manages episodic and stable chronic conditions commonly seen in primary health care, consulting with CHC physicians
  • Provides supportive counselling which may include preventative counselling, brief supportive counselling or short-term crisis management
  • Writes prescriptions and orders laboratory tests within the RN(EC) scope of practice
  • Provides pre and post-natal care which includes collaboration with CHC physicians, obstetricians, "maternity care" physicians, midwives or health care providers chosen by the client, as needed
  • Makes home visits to clients who live within the catchment area as necessary
  • Makes internal referrals to other SRCHC staff as appropriate
  • Refers clients to partner organizations, specialists, hospitals or community resources as

appropriate

  • Participates in case conferences with interdisciplinary team members, partner organizations, and other providers to ensure coordination of services to the client
  • Ensures that standards of client care are maintained according to accepted College of Nurses standards, the Regulated Health Professions Act, and the policies of SRCHC
  • Performs necessary back-up clinical functions for other nursing staff in their absence
  • Provides care at outreach clinics as needed
  • Integrates health promotion principles into interactions with individuals, families or groups
  • Assists and works with community groups in the development of community outreach programs as directed
  • Participates in the development of, and gives support to, policies/strategies which are in the interest of public health and which support the recognition of the determinants of health, recognizing the social and structural inequities that shape access to health and healthcare
  • Incorporates health care advances into practice and adjusts practice to the changing demographics and evolving needs of the community
  • Maintains complete and accurate records sufficient for timely communication with other professionals
  • Provides statistical information as required
  • Participates in the development of clinical policies and procedures and medical directives as required
  • Participates in planning and evaluation of clinical programs and activities and SRCHC program reviews as required
  • Contributes to the medical and nursing team's use of research methodologies and findings in clinical practice, and to monitoring and evaluation
  • May perform clinical management tasks such as infection control, sterilization procedures and maintenance of clinical equipment and supplies
  • All other duties as required

Qualifications:

  • Graduate degree in nursing from a recognized university
  • Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario and certificate of continuing competence
  • 3-5 years of experience in a community setting or a combination of community experience and relevant hospital or public health nursing
  • Thorough current nursing knowledge and proficiency in conducting a comprehensive health history, physical assessment and construction of a nursing management plan
  • Experience working from a strength-based orientation and trauma informed lens
  • Skills and experience working with people who use drugs, people with complex trauma, and people with mental health considerations
  • Overdose response experience required
  • Crisis de-escalation experience required
  • Knowledge and experience advocating for systemic change related to drug use, sex work, incarceration, harm reduction services, Hepatitis C, HIV and overdose prevention
  • Demonstrated ability to work effectively in a multi-disciplinary environment
  • Demonstrated commitment to and knowledge of community-based health care
  • Proficiency in the use of computer technology and various software applications
  • Strong verbal and written communication skills
  • Excellent interpersonal skills
  • Excellent organizational and administrative skills
  • Strong interest in issues affecting people who are homeless, living with a mental illness and/or substance use, and complex health needs
  • Respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
  • Fluency in a language relevant to the South Riverdale Community is a benefit, in addition
