General Description:
Under the direction of the Home Community Care Coordinator, the Licensed Practical Nurse (LPN) supports and enhances the delivery of Community Health and Home Care programs within the Sipekne'katik community. The LPN provides client-centered care, health education, and nursing services that promote wellness, prevent illness, and support Elders and community members in maintaining optimal health at home.
The position requires a collaborative approach with other health professionals and adherence to the standards, ethics, and regulations governing practical nursing in Nova Scotia.
Responsibilities:
Client Care and Nursing Support
- Conduct home visits with Elders enrolled in the Visiting Elder Program and other community members as assigned.
- Perform nursing procedures and client education in accordance with individualized care plans.
- Accompany Registered Nurses on initial client assessments as needed.
- Provide professional foot care services to both diabetic and non-diabetic clients.
- Reinforce health education and program goals with clients and families to encourage participation in care.
- Observe and document client condition and report significant changes or concerns promptly.
Program and Clinic Support
- Assist Community Health Nurses with Adult Health and Lifestyle Clinics, including registration, screening, immunizations, and health promotion activities.
- Participate in health education and awareness programs that encourage wellness within the community.
- Support the coordination and preparation of community health events, including the organization of materials and scheduling.
- Direct inquiries about programs and services to the Home and Community Care Coordinator or appropriate staff.
Documentation and Reporting
- Complete accurate and timely charting of client care and all nursing procedures performed.
- Prepare and submit incident, accident, and follow-up reports as required.
- Maintain confidentiality and secure storage of all client health information and documentation.
- Assist with maintaining and updating client records and home visit schedules.
Equipment and Safety
- Assist in maintaining, cleaning, and returning medical equipment after use.
- Follow infection prevention and control protocols during all clinical and home care activities.
- Identify and report environmental or safety hazards observed in clients' homes.
- Ensure compliance with the LPN Act, Regulations, Code of Ethics, and Standards of Practice as defined by the College of Licensed Practical Nurses of Nova Scotia (CLPNNS).
Collaboration and Communication
- Work collaboratively with the Home and Community Care Coordinator and other health professionals to ensure high-quality, coordinated client care.
- Participate in interdisciplinary case reviews and care planning discussions.
- Maintain positive, respectful communication with clients, families, and team members.
- Promote cultural safety and respect for Mi'kmaq traditions and community values in all interactions.
Qualifications and Experience/Skills:
- Current and valid registration with the College of Licensed Practical Nurses of Nova Scotia (CLPNNS).
- Demonstrated ability to maintain confidentiality, professionalism, and cultural sensitivity.
- Current Standard First Aid and Basic Life Support (BLS) certifications.
- Current WHMIS certification.
- Willingness to participate in ongoing professional development and nursing-related training.
- Additional training or certification in Foot Care, Mental Health First Aid, and COHI (Children's Oral Health Initiative) considered an asset.
- Strong interpersonal and communication skills.
- Ability to work both independently and collaboratively within a multidisciplinary team.
