Job Summary:

Identifies the person's strengths, needs, preferences and risks. Develops and implements short-term plans that include goals, objectives & timelines that align with the overall Service Plan. Uses basic counselling techniques to provide emotional support and counselling to the person being supported in the Resource Home. Provides skill building using various experiential strategies in areas that align with the person's goals, preferences, and risks.

Qualifications: Education, Training & Experience

  • • Diploma in related human/social service field.
  • • Two (2) years of recent related experience.
  • • Or an equivalent combination of education, training, and experience.

Key duties and responsibilities:

  • Assesses the person's strengths, needs, preferences and risks by using various strategies, and formalized tools, conducting interviews, and meeting with the person's support network.
  • Review information with the Program Manager to identify strengths, needs, preferences & risks. Develops a plan with the person being supported, that includes goals, objectives & timelines to address short-term specific needs and risks.
  • Plans and engages, using a trauma information approach, in one-to-one emotional support and emotional regulation sessions using basic counselling techniques such as active listening, conflict resolution, paraphrasing, asking clarifying questions and summarizing.
  • Supports the person served to develop a supportive social network that promotes healthy choices and relationships.
  • Works with the Program Manager and in partnership with the person, develops and documents safety plans to address high-risk situations, choices, relationships and behaviours. Provides life skills training using a trauma-informed approach that aligns with the person's needs, risks, preferences, and goals. This may include but is not limited to supporting the person to understand and manage their emotions and finding new alternatives to express feelings and emotions.
  • Uses person-centred activities and recreation to support emotional regulation, achieve goals, and learn self-management.
  • Recognizes, analyzes, and takes a proactive approach to potential emergency situations, using crisis intervention training and strategies to ensure no harm comes to the person, staff and/or the public. Reports crisis to supervisor and completes and submits a Critical Incident form.
  • Evaluate the effectiveness of the various interventions, report progress and discuss related concerns with the Program Manager, to resolve and/or find new potential solutions to challenges and risks to support the person to achieve their goals and objectives.
  • In collaboration with the Program Manager, provide referrals to community service providers, resources, and professionals.
  • Provides emotional support to the person served to assist with emotional regulation, de-escalation and/or debriefing of an emotional situation.
  • Assists the person served with activities of daily living such as feeding, lifts, transfers, hygiene, grooming and toileting.
  • Administers, records and stores medications in accordance with established policies and CARF standards.
  • At the direction of the Program Manager, schedule, transport and assist the person served with medical and dental appointments.
  • In collaboration with the Program Manager, works closely and supports the person's served network, including family, caregivers and others identified by the person.
  • At the direction of the Program Manager, liaison with other agencies, professionals, government officials and the community.
  • Performs other related duties as required

Training

You will participate in a comprehensive 11-day training program, followed by three (3) shadow shifts designed to fully prepare you to support youth and adults in a safe, effective, and productive environment. Throughout your tenure, you'll have opportunities for internal growth, advancement, and continued professional development through additional training and education offered internally and by third-party providers.
