INTRODUCTION:

New Path Youth and Family Services is an accredited children's mental health agency providing a range of compassionate, innovative and accessible services for children, youth and their families throughout Simcoe County. New Path respectfully acknowledges we are situated on the traditional land of the Anishnaabeg people which include the Odawa, Ojibwe, and Pottawatomi Nations collectively known as the Three Fires Confederacy.

PURPOSE OF POSITION

To provide evidence based/informed programs to students and their families in their home, school and community settings. Provide mental health guidance and support to school personnel and to cross-sector community agencies to increase their capacity to respond effectively to children and youth with mental health needs. 

JOB DUTIES AND RESPONSIBILITIES:

  1. Meet with students and/or families in school, at home or in the community to assess and provide evidence based/informed programs to overcome emotional, behavioural and social difficulties.
  2. Participate in case planning meetings, as necessary, to provide input into the treatment goals and objectives, plans for service, and programs achieving the goals of clients.
  3. Act as a liaison between the parents and the school.
  4. Provide evidence based/informed programs in schools in accordance with specific treatment or individual education plans of students.
  5. Provide guidance and support to school personnel (e.g. classroom teacher, SERT) with respect to strategies around behaviors, issues and needs of a specific student.
  6. Collaborate with school and community personnel to help develop programs and strategies that meet the identified individual or family needs.
  7. Provide leadership in the development of "wraparound type teams" composed of school and community professionals dependent on the needs of each individual and family on the assigned caseload.
  8. Prepare and deliver in-school school presentations on behavioural interventions and strategies.
  9. Act as case manager for assigned clients.

QUALIFICATIONS:

  1. Child & Youth Worker diploma or equivalent.
  2. A minimum of 2 years experience working with children and youth with emotional, behavioural or social challenges using evidence based/informed programs.
  3. An additional 1 year of experience working with students in a school setting who are experiencing social emotional and behavioural challenges.

  10. Competency and effective use of database and Microsoft Office systems.
  11. Confident, articulate and professional when communicating verbally and in writing.
  12. French language is an asset.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

  1. Must be available to provide 4 hours per week of direct service to clients after 5 p.m. (evening appointments) to accommodate client and family schedules.
  2. This position is primarily performed in office/on-site to support the in-person meeting preferences of our clients and families. Some administrative tasks (non-client facing work) can be requested to be completed remotely (Work From Home).
  3. Prolonged periods of sitting and working on a computer.
  4. Willingness to travel to other agency offices and the community using personal vehicle. Must have valid driver's license with minimum G2.
