About ISO-Santé

ISO-Santé is a multidisciplinary rehabilitation firm specializing in both physical and mental health. The organization serves individuals as well as institutional clients such as the SAAQ, CNESST, and private insurers.

ISO-Santé is also actively involved in health promotion and the prevention of musculoskeletal and mental health disorders (MSDs and MHDs) in the workplace, collaborating with numerous corporate clients across Quebec and even across Canada.

This position is being offered to support a national client in managing disability cases.

Position Description

The Disability Case Manager plays a key role in the proactive management of cases involving workers who are on leave or undergoing rehabilitation. He or she coordinates interventions among the client, healthcare professionals, employers, and insurers to promote a safe, timely, and sustainable return to work.

Main Responsibilities

  • Manage a portfolio of disability cases (short- and long-term, CNESST, WSIB, private insurance, or self-insured programs).
  • Assess clients' health, support, and rehabilitation needs.
  • Review and analyze medical and professional reports to develop individualized management plans.
  • Ensure effective liaison and communication among all involved stakeholders.
  • Develop, update, and monitor Return-to-Work (RTW) or Stay-at-Work plans.
  • Maintain regular communication with clients and partners to ensure a professional and compassionate follow-up.
  • Document all actions and interventions in the case management system in accordance with internal policies.
  • Identify medical, organizational, or psychosocial barriers that may delay work resumption.
  • Produce clear, concise, and solution-oriented written reports.

Skills and Core Competencies

  • Excellent written and verbal communication skills in French and English (bilingualism required).
  • Knowledge of CNESST regulations and, ideally, WSIB procedures elsewhere in Canada.
  • Strong analytical, synthesis, and prioritization skills.
  • Empathy, active listening, and a client-centred approach.
  • Strong sense of interdisciplinary collaboration and coordination.
  • Administrative precision and respect for confidentiality.
  • Autonomy, sound professional judgment, and proactive problem-solving abilities.

Qualifications

  • College or university degree in a field related to disability management, workplace health, or another relevant discipline.
  • Minimum of 2 to 5 years of experience in disability case management, occupational health, insurance, or rehabilitation (CNESST, group or private insurance).
  • Knowledge of WSIB is considered an asset.
  • Solid understanding of return-to-work processes, CNESST policies, and rehabilitation planning.
  • Proficiency with computer tools (Microsoft Office Suite, case management software).
  • Strong knowledge of the Quebec healthcare and insurance network (major asset).
