COMMUNITY SUPPORT WORKER
About the role
As our Community Support Worker, you will work as part of a vibrant team to provide care and support to people living in staffed homes within their community. You'll encourage and empower the people we support to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work both independently and collaboratively as a member of a multidisciplinary team will allow you to influence your team's success.
Core Responsibilities
Ensure the supported persons' rights and choices are respected and advocate as neededInteract effectively with people we supportEmpower the people we support to be full citizens of their communityParticipate in the development, implementation, and documentation of Personal Service PlansFoster positive relationships with persons' family members, professionals, co-workers, and members of the communityAssist with activities of daily living and medication administrationExcellent understanding of behaviour management principles and strategies including mental health needsRecord the daily activity and personal health information (PHI) of the people we support on our case management databaseTo work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping dutiesYou will have
Grade 12 educationCommunity Support Worker Diploma or equivalentMinimum 2 years of experience in community social services/health careCurrent Emergency First Aid/CPR and Food Safe certificatesPrevious behaviour management course/trainingAbility to effectively communicate (verbal & written) with team members, professionals, families, and community membersNon-verbal communication skillsValid class 5 driver's licence Satisfactory Driver's AbstractCriminal record clearance for children and vulnerable adultsTB TestModerate computer literacyEligibility to work in CanadaWhy join Pacific Coast Community Resources?
