The Project Lead, Digital Health provides project management and implementation support for digital health initiatives at Grand River Community Health Centre. Working under the supervision of the Director of Quality and Primary Care Integration, the Project Lead is responsible for coordinating adoption, education and evaluation of digital health tools, and ensuring projects are delivered on time, within scope.
This part-time role is ideal for a motivated individual with experience in project planning, coordination, implementation and monitoring specifically related to digital health tools in a primary health care setting. The successful candidate will be highly organized, solutions focused and have the ability to support staff through a successful change in process. This position is ideal for a self-motivated individual seeking flexible work while making a meaningful contribution to healthcare system transformation.
Key Responsibilities
- Coordinate the planning, implementation, and evaluation of digital health projects e.g. Strello
- Support staff adoption of digital tools by providing training, developing resources, and offering ongoing, timely assistance.
- Support staff using change management strategies
- Engage and collaborate with frontline staff to integrate digital tools into clinical workflows.
- Monitor project timelines, deliverables, and risks, and escalating issues where necessary.
- Develop risk mitigation strategies.
- Collect and report data on adoption, user satisfaction, and impact on workflows.
- Ensure digital health initiatives reflect principles of equity, diversity, inclusion and cultural safety.
Qualifications
- Post-secondary degree/diploma in a Health, Project Management or related field.
- Minimum 3-4 years of experience supporting digital health projects, technology adoption, and workflow integration.
- Demonstrated facilitation training and change management skills with frontline healthcare teams.
- Experience in project coordination and implementation, including timelines, deliverables, and reporting.
- Demonstrated problem solving and critical thinking skills.
- Knowledge of Ontario's health system, community health, and primary care environments are an asset.
- Excellent communication, presentation, interpersonal, and problem-solving skills.

Pay: $33.83-$40.41 per hour
