1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
2. Baccalaureate or Masters degree in Social Work from an accredited School of Social Work
3. Excellent knowledge of community support and placement services
4. Minimum 2 years recent experience working with a stroke population within the last 2 years
5. Knowledge of Healthcare Consent Act, Substitute Decisions Act, Child and Family Services Act
6. Excellent crisis intervention and communication skills
7. Proven ability to work as a member of interdisciplinary healthcare teams, working both independently and collaboratively in a variety of settings
8. Proven ability to organize and prioritize patient caseload across several programs
9. Hospital-based clinical experience in an acute care and rehabilitation setting

Please note that all Social Worker postings are posted at the Bachelor of Social Work (BSW) rate (unless MSW is required). Should the successful applicant possess a Masters of Social Work (MSW) and provide proof they will be adjusted to the MSW rate to correctly reflect the level of education in accordance with the letter of understanding.

This position is temporary until May 2026.

The Integrated Stroke Program includes the Acute Site (7 South) Rehabilitation (B2 North) and St. Peter's Hospital. This position will provide service across the continuum for 7 South and B2 North Rehabilitation. The Neurosciences Program on 7 South provides inpatient service across the continuum to individuals who suffer from strokes and stroke related diagnosis.
