One that centers Indigenous voices and works alongside partners to reduce isolation and foster inclusion?The Hamilton Council on Aging (HCoA) is home to the Dementia Empowerment Network (DEN), a growing movement of peer-led groups created by and for people living with dementia, as well as those supporting their journey (care partners). These groups reduce social isolation, build friendships, and provide meaningful opportunities for leadership and connection.Through a new partnership with Six Nations of the Grand River and De dwa da dehs nye>s Aboriginal Health Centre, we are expanding this model to support Indigenous older adults living with dementia and their care partners.Outreach Coordinator (Part-Time, 25 hrs/week)Location:Hamilton & Six Nations of the Grand River regionTerm:Start immediately until March, 31, 2029Wage:$33/hr, Part-time, 25 hours per weekReports To: HCoA Board Representative & Project ManagerI. Scope and Summary of Primary ResponsibilitiesThe Outreach Coordinator plays a key role in building and maintaining strong, trust-based relationships between our project, community partners, and people living with dementia and their care partners.This position is highly community-facing. The Outreach Coordinator will often be "out in the field" attending events and supporting peer-led groups. They are responsible for keeping groups connected and engaged, supporting member recruitment, and leading creative outreach efforts through social media and marketing. The role is highly flexible, with responsibilities that can change from day to day depending on community needs.Because of the unpredictable nature of community work, flexibility, initiative, and strong interpersonal skills are essential.Regular access to a car is required,as frequent travel across Hamilton and Six Nations will be part of the role. Training on dementia is an asset but can be provided.II. Essential FunctionsThe position entails, but is not limited to, the following responsibilities:Community EngagementBuild and maintain strong relationships with group members, care partners, and volunteersAct as a liaison to the public, including individuals interested in joining, families, and partner organizationsIdentify and attend public events, ceremonies, and gatherings to raise awareness and recruit new participantsSupport individuals living with dementia and/or their care partners in joining existing peer-led groups or starting new onesParticipate in monthly project Advisory Council meetings and contribute community updatesGroup & Volunteer SupportRequest regular updates from groups/volunteers and stay in close communicationAttend group meetings and step in as needed (e.g., if a volunteer is absent)Provide ongoing, as-needed support to volunteers and peer-led groupsLiaise with the Alzheimer Society of BHNHH to coordinate volunteer recruitment and trainingCollaborate with partner organizations (SNGR & De dwa da dehs nye>s) to recruit and support new members, care partners, and Advisory Council representativesMarketing & CommunicationsDevelop and distribute outreach materials (digital and print)Distribute promotional materials in community spacesManage and maintain social media accounts (LinkedIn, Facebook, Instagram) and update the website with news and opportunitiesCreate engaging posts, share updates, and interact with online communities to support recruitment and awarenessDocumentation & SustainabilityDocument project learnings, stories, and outcomesLead the development of knowledge transfer tools (e.g., guides, handouts, resources) to support group sustainability and community adaptationContribute insights to project evaluation, strategic planning sessions, and reportingIII. QualificationsKnowledge and Skill RequirementsExcellent interpersonal and communication skills, with a focus on relationship-buildingStrong community connections or familiarity with Indigenous communities in Hamilton and Six NationsFlexible, adaptable, and comfortable responding to last-minute changes or urgent needsOrganized and reliable in managing multiple relationships and responsibilitiesCollaborative team player who values input from others, is open to feedback, and works well in partnership with colleagues, community members, and advisory groupsCreative thinker with experience in communications, outreach, or social mediaProficiency with SharePoint, Canva, and social media platforms (LinkedIn, Facebook, Instagram)Experience and EducationExperience in community engagement, outreach, volunteer coordination, or related fields (required)Post-secondary education in communications, social sciences, community development, or equivalent lived/professional experienceWork EnvironmentHybrid (working from home and travelling across the region)Travel within Hamilton and Six Nations required (regular access to a vehicle is essential)Ability to work flexible hours, including some evenings or weekendsValid driver's license requiredApply NowIf this sounds like the role for you, we'd love to hear from you.
