The Rexdale Community Health Centre (RCHC) is a multi-service centre providing primary health care, health promotion and community support services, including settlement and community development activities for families, children, youth, adults and seniors of diverse backgrounds.
RCHC is currently recruiting two social workers to join our Interprofessional Primary Care Team (IPCT). This team is part of a broader initiative to expand access to comprehensive, team-based care in the community.
As a Social Worker, you will play a vital role in delivering high-quality, client-centred wrap-around support to individuals and families in marginalized communities. Working within our IPCT, you will provide assessments, culturally appropriate support, such as system navigation and timely referral to appropriate services, as well as short-term intervention. You will also provide counselling and case management.
This position requires a strong commitment to culturally competent care, collaboration with an interdisciplinary team, and a passion for working with underserved communities. The Social Worker will deliver care both at community partner sites and on board our mobile "Wheels to Wellness" bus, bringing comprehensive care and wrap-around services directly to clients where they live, work, and gather.
Responsibilities:
- Engages in short-term assessment, referral, and advocacy activities
- Acts as a resource to and advocate for individuals, groups, and the community.
- Provides risk assessment, personal and practical support, information, referral and advocacy with individuals.
- Assesses client status by gaining an understanding of social determinants of health.
- Acts as a client advocate on individual and systemic issues.
- Provide crisis intervention services both over the telephone and in person
- Contributes to chart documentation, may participate in chart reviews and case conferences, reflective practice meetings/discussions and makes referrals.
- Participates in the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, families and the community.
- Acts as a consultant on resources for other service providers in the Centre and in the community.
- Liaises and collaborates with staff, teams and other community service providers as appropriate to provide service and make referrals.
- Assists clients and the community to build support networks.
- Ensures accountability by providing statistics and reports of activities, as required, and keeping client records, as required, within the safeguard of confidentiality.
- Provide individual and/or group therapy using CBT, DBT and other evidence-based approaches.
- Develop group therapy programs, and facilitate group therapy sessions.
Qualifications and Assets:
- University degree in social work, counselling, psychology or related field;
- Current Registration with an appropriate regulatory college, e.g. OCSWSSW, CPO, CRPO.
- Minimum three years of experience in intake and assessment within a mental health, or clinical setting
- Minimum five years counseling/therapy experience, preferably in community based health setting with youth and their families.
- Proven skills in psychosocial assessment, counseling/therapy, crisis intervention, case management and referrals
- Demonstrated skills and experience in working with racialized communities
- Demonstrated commitment to and lived experience of racialized communities
- Knowledge of community resources
- A deep fundamental and comprehensive knowledge of the unique challenges faced by racialized communities across the social determinants of health.
- A commitment to interdisciplinary team approach to coordination of health and social services
- Proficiency in current assessment, counseling/therapy and psychosocial counseling techniques for working with individuals, groups and communities
- Demonstrated skills and experience in the assessment and treatment of substance abuse and/or mental health treatment inclusive of individual and group counseling.
- Strong communication skills (verbal and written) combined with computer literacy are required.
- Ability to negotiate, communicate and maintain a supportive, trusting relationship with those being served.
- Experience in program development, implementation, and evaluation.
- Knowledge of community-based health services
- Strong and demonstrated ability to form relationships with individuals while maintaining healthy boundaries
- Caseload management skills, including developing action plans for clients, monitoring client progress and maintaining records
- Working knowledge of the Ontario Mental Health Act
- Knowledge of Personal Health Information Privacy Act, Consent of Information / Records of Information and Medical Records Management
- Knowledge of the Rexdale community
- Bilingual in French and English considered a strong asset
- Must be able to provide a vulnerable sector security clearance
NOTE:
- A criminal record check will be a required
- Rexdale Community Health Centre has a mandatory vaccination policy in place
Salary Range: $65,327 to $78,704 annually (based on experience) Plus HOOPP & Benefits
Hours per week: 35 hours
