Job Title: Bilingual Nurse Practitioner

Location: Hamilton – Onsite

Full-time – Permanent – 35 Hours per week

Monday to Friday – Flexible working hours between 8:00am – 5:00pm

Our client is seeking a
Bilingual Nurse Practitioner (French/English)
to join their primary care team in Hamilton. Reporting to the
Primary Care Services Coordinator
, you'll provide comprehensive, client-centred health care focused on
health promotion, illness prevention, and chronic disease management
.

You'll work both independently and collaboratively with physicians and other healthcare professionals to deliver quality care that supports the well-being of our community.

What You'll Do:

  • Provide primary health care services to clients of all ages.
  • Assess, diagnose, and manage common health conditions independently or with physician support.
  • Engage clients in identifying health risks, setting goals, and choosing care plans.
  • Prescribe and interpret diagnostic tests as needed.
  • Maintain accurate, confidential client health records.
  • Support clients with stable chronic conditions and ensure appropriate follow-up care.
  • Refer clients to internal programs and community resources.
  • Participate in policy and procedure development, quality improvement, and data collection.
  • Collaborate with an interdisciplinary team to ensure coordinated, holistic care.

Qualifications:

  • Bilingual (French & English)
    – spoken and written
  • Bachelor of Science in Nursing
    and
    Nurse Practitioner Certificate
    from a recognized university.
  • Good
    standing with the
    College of Nurses of Ontario (CNO)
    .
  • Excellent clinical judgment, problem-solving, and communication skills.
  • Ability to work both independently and within a
    multidisciplinary team
    .
  • Proficient in standard computer applications and electronic health records.
