You will play a vital role in helping to shape meaningful change in respiratory care across the health system while gaining valuable experience in program coordination, stakeholder engagement, and clinician education.   Reporting to the Senior Manager, Clinical Practice Transformation, the Lead, Clinical Practice Transformation plays a pivotal role in advancing the Lung Health Foundation's efforts to improve the prevention, diagnosis, and care of chronic respiratory conditions in Canada. As a key member of the Programs & Public Affairs team, the Lead is responsible for the design, delivery, and continuous improvement of quality improvement initiatives focused on COPD, asthma, and related respiratory conditions.   Reporting to the Senior Manager, Clinical Practice Transformation, the Lead will support several cornerstone initiatives, including the Primary Care Asthma and COPD Program (PCAP), a province-wide model that equips primary care providers to deliver high-quality, evidence-based respiratory care. The role will also support related quality improvement efforts, clinical tool and resource development, and knowledge translation activities aimed at reducing care gaps, preventing avoidable hospitalizations, and improving health outcomes for people living with COPD and asthma.   The Lead will collaborate closely with healthcare providers, policymakers, researchers, and individuals with lived experience to ensure all programs are grounded in the best available evidence and reflect real-world needs. By building strong partnerships and driving innovation, the Lead will help shape the future of respiratory care across Ontario and Canada. The ideal candidate brings a strong background in chronic disease management, a commitment to health equity, and a passion for improving the lives of patients and communities.   Key areas of focus include:
  • Supporting the execution of advocacy strategies across COPD and asthma
  • Assisting with stakeholder engagement, including policymakers, clinicians, and people with lived experience
  • Contributing to evidence-informed program delivery and evaluation
  • Helping ensure advocacy and program initiatives are aligned with strategic priorities and grounded in equity
  • Leverages data, analytics, Quality Improvement (QI) methodology and best practice evidence to improve client care and service delivery.
  RESPONSIBILITIES   Programmatic Support
  • Support the planning, implementation, and evaluation of LHF programs related to COPD, asthma, and chronic respiratory care.
  • Translate research and best practices into accessible, impactful patient- and provider-facing programs.
  • Help support the identification and response of service gaps through the development of innovative initiatives grounded in person-centered care.
  • Collaborate with cross-functional teams to ensure alignment between programs, advocacy, and public education.
  Partnership Development
  • Engage clinical stakeholders, researchers, health organizations, and community groups to inform program design and build momentum for system-level change.
  • Support advisory committees, working groups, and consultations that ensure programs reflect real-world needs and lived experiences.
  • Strengthen LHF's network of partners supporting respiratory care in primary care and community settings.
   Quality Improvement & Evaluation
  • Monitor program performance, track outcomes, and recommend improvements based on data and stakeholder feedback.
  • Lead environmental scans, jurisdictional analyses, and literature reviews to ensure programs reflect current evidence and emerging trends.
  • Collaborate on funding applications, knowledge translation, and strategic planning to expand LHF's impact in chronic disease management.
  Diversity, Equity, and Inclusion (DEI) Skills
  • Apply a health equity lens to all aspects of program design and evaluation.
  • Ensure culturally safe, inclusive, and accessible programs for equity-deserving populations.
  • Champion principles of co-design and person-centeredness in collaboration with individuals affected by asthma and COPD.
  • Incorporate anti-oppression frameworks and the social determinants of health into program planning.
  Lung Health Foundation Champion
  • Contribute to additional programs or initiatives based on emerging priorities
  • Help grow and maintain LHF's network of clinical and community partners
  • Support cross-functional duties aligned with organizational goals
  • Assist in external communications and advocacy efforts, including media engagement in collaboration with the Marketing and Communications team
  QUALIFICATIONS
  • A degree in public health, health sciences, life sciences, or a related field such as a Bachelor of Arts is required; graduate-level training is considered an asset.
  • At least 4 years of experience in:
  • Chronic disease management, health promotion, or public health program delivery
  • Healthcare quality improvement, patient education, or system navigation
  • Stakeholder engagement and/or co-design
  • Experience working in respiratory health, primary care, or health systems is a strong asset.
  • Strong understanding of evidence-based practice, quality standards, and patient-centered care.
  • Experience conducting and interpreting quantitative data analyses to inform decision-making, program planning and evaluation.
  • CRE designation considered an asset.
  • Fluency in French is considered an asset.
