Position Summary

The Social Work (SW) & Community Health Navigator (CHN) Supervisor works in direct collaboration and partnership with the Primary Care Manager (PCM) of Behavioural Health to support a high-functioning primary care team and the delivery of Social Work and CHN services. The Supervisor is responsible for the practice leadership, performance management, coaching, development and frontline clinical support to the SW and CHN teams. This role also provides supportive leadership to co-location and community/zone SW and CHN programming.

The Supervisor participates in development, monitoring, evaluation, and continuously improve the delivery of programs & services within their program area and provide integral work to advancing quality improvement projects, initiatives and data driven decision making.

Key Responsibilities

People Management

  • Provides day-to-day supervision to team members including practice guidance, clinical support, scheduling, and timecard approvals.
  • Facilitates and leads regular team meetings, 1:1s, and other meetings/committees under their assignment.
  • Supports practice excellence, professional growth and development through mentorship and coaching.
  • Supervises team members in adherence to the Collective Bargaining Agreement (CBA) and HR processes.
  • Responsible for the recruitment, onboarding/offboarding, orientation and training of new hires and practicum students.
  • Responsible for conducting probationary and annual performance reviews.
  • Provides support, coaching and performance management to direct reports to ensure performance standards and accountabilities are understood and met.
  • Conducts regular audits to ensure level of continued competency to provide active feedback and appropriate training to staff.
  • Provides "super user" leadership and coaching to the team to support competency with MPCN systems.
  • Creates opportunities for recognition and visibility of team members success and contributions.
  • Works in close partnership with the PCM to support early identification and resolution of program level or personnel concerns.

Internal and External Relationship Management

  • Provides visibility and clinical representation of their teams at an organizational level, supporting the multidisciplinary teams with program related questions and supporting opportunities collaborative care.
  • Works in direct partnership with the PCM with clinical project work, team and professional development, and quality improvement initiatives.
  • Maintains relationships with Senior MOAs and/or MOA Supervisor to identify and support opportunities to improve communication and efficiencies with the PHP programs & services.
  • Liaise with member physicians and external community stakeholders to develop relationships, address concerns, and support the success of the teams.
  • Fosters relationships with internal and external stakeholders where required.

Training, Education and Orientation

  • Coordinates and supports ongoing professional development and learning opportunities.
  • Works in collaboration with the PCM to identify and coordinate shared learning opportunities for the team.
  • Develops and facilitates training and/or information sessions and practice support materials as required, based on identified operational and educational needs.

Policies, Procedures, and Control

  • Works in collaboration with the PCM to ensure that the appropriate controls, systems and procedures are developed and maintained to ensure accuracy, integrity and privacy of clinic and patient information.
  • Supports the ongoing alignment of MPCN services and professional development to the practice and competency requirements of the program's discipline specific regulatory bodies.
  • Maintains privacy and confidentiality by understanding the Health Information Act (HIA) and the Privacy Information and Protection Act (PIPA) and their applications to the programs/services.

Change Leadership and Quality Improvement

  • Participates in the regular review and monitoring of key performance indicators and works with the team and PCM to identify and address barriers to accurate and timely data entry and documentation.
  • Collaborates with the PCM in utilizing change management principles to successfully implement and champion organizational and program level initiatives.
  • Supports the PCM in maintaining a culture of continuous quality improvement and to champion initiatives which improve patient and provider care experiences and operational efficiency.
  • Maintains a front-line service focus to ensure early identification of emerging trends, practice challenges and opportunities.
  • Maintains ongoing awareness of the local and system context impacting mpcn programs and services.

Program Planning and Operations

  • Maintains clinical competency and expertise through direct support of the teams to address emergent/urgent patient issues, pilot and/or refine process or practice changes, and support the resolution of patient and provider level concerns in consultation with the PCM
  • Provides day-to-day leadership including scheduling, communications coordination.
  • Supports the PCM in planning and development activities, clinical projects, and areas of inter-professional collaborative practice.

Qualifications & Requirements

Education & Licenses

  • Bachelor's Degree in Social Work is required.
  • Master's Degree in Social work preferred.
  • Active registration and in good standing with the Alberta College of Social Workers (ACSW)

Experience

  • Minimum of 5 years of experience in a healthcare-based or related non-profit community setting.
  • Prior supervisory experience is an asset.
  • Experience working with diverse and vulnerable populations an asset.
  • Primary Care and PCN experience in an asset.

Knowledge, Skills and Abilities

  • Excellent interpersonal and verbal communication skills; ability to effectively interact with diverse professional and patient stakeholders and navigate challenging or delicate conversations with tact and diplomacy.
  • Strong critical thinking and problem-solving abilities; able to work well under pressure.
  • Comprehensive understanding of the primary health care and healthcare system in Alberta; knowledge of primary care service delivery and primary care networks an asset.
  • Able to thrive in a dynamic and integrated team environment through strong relationship building skills, group problem solving abilities, and shared information and responsibility.
  • Strong written communication skills, including the ability to develop content for both professional and patient/service user audiences.
  • Good facilitation and presentation skills; foundational knowledge of adult learning principles.
  • Knowledge of, and experience with resources related to basic needs, chronic disease, and social benefits programs at the community, provincial, and federal level.
  • Demonstrated understanding of intercultural practice and experience working with diverse populations and commitment to diversity and inclusion.
  • Strong understanding of social determinants of health and demonstrated experience with addressing barriers to wellbeing in diverse and vulnerable populations.
  • Demonstrated excellence in interpersonal, verbal, and supportive communication skills.
  • Strong time management skills and flexible working style; ability to adapt and thrive in an evolving health care environment.
  • Computer literacy including, Microsoft Office suite, Outlook calendar/email management, and Electronic Medical Records (EMRs)

  • Occasional evening and weekend hours may be required, including the possibility of rotating on-call support
