The Opportunity

We are seeking a Nurse Practitioner (NP) with a strong interest and experience in diabetes management and primary care to join our collaborative, multidisciplinary team. This role is ideal for an NP who thrives in a supportive environment where they can truly make a difference in patients' lives while helping shape the future of community healthcare.

What You'll Do

  • Provide comprehensive primary care services, including assessment, diagnosis, treatment, and follow-up.
  • Lead and manage care for patients living with Type 1 and Type 2 diabetes, including medication management, insulin initiation/titration, lifestyle support, and complication monitoring.
  • Collaborate with physicians, pharmacists, dietitians, and allied health providers to ensure seamless, team-based care.
  • Participate in developing and improving chronic disease programs within Care2Talk Health.
  • Deliver both in-person and virtual care appointments, ensuring accessibility and continuity for patients.
  • Contribute to the supportive, patient-first culture we are building across our network.

What We're Looking For

  • Current registration (in good standing) as a Nurse Practitioner with the BC College of Nurses and Midwives (BCCNM).
  • Demonstrated experience or specialized training in diabetes care and chronic disease management.
  • A strong commitment to holistic, patient-centered care.
  • Comfort with electronic medical records (EMRs) and virtual care platforms (Accuro experience an asset).
  • Excellent communication, teamwork, and leadership skills.
  • Flexible and adaptable, with a passion for innovation in healthcare delivery.
