What you'll be doing:
Reporting to the Vice President, Quality & Clinical Services, the Senior Manager, Long Term Care Quality & Risk will be responsible for leading, developing, and implementing efficient evidenced based processes and services related to Quality & Risk management nationally within the
organization, and in keeping with relevant legislative standards and regulatory bodies. As part of
the long-term care support office team, the Senior Manager, Long Term Care Quality & Risk
collaborates with regional partners and community leadership to address priority initiatives,
sustain best practices and identifying risk/ mitigate
.
- Manages Accreditation Lead and provides direction and guidance to all direct
- Provides subject matter expertise and tactical support to community leadership
to ensure effective and quality execution of Quality Improvement Plans (QIP) and
Continuous Quality Reports
- Monitors, reports and responds to clinical/non-clinical indicator performance
outcomes in collaboration with community leadership and regional partners.
- Coordinate and facilitate the scale and spread of Sienna initiatives and
improvements across the communities.
- Develop and leverage auditing systems and processes as per the quality and risk
management program to ascertain compliance, system effectiveness, identify
improvement opportunities and potential risk, validate improvement/change
management initiatives
- Analysis, tracking, and trending of quality and risk data collected across Sienna
Senior Living homes, with follow-up and development of strategies for
management of identified risks.
- Develops policies and procedures for the Quality and Risk Manual. Oversees the
quality and risk framework for Long-term Care.
- Support in the development of the key performance indicators for Long-term Care.
- Support with the development of compliance action plans as assigned and validate
compliance achieved where needed.
- Apply and execute quality improvement methodologies to advance performance
such as root cause analysis, process mapping, appreciative inquiry, prospective
analysis (FMEAs), retrospective analysis, PDSA cycles
- Provide direction, plans of action and support to those homes identified as "high"
risk through the application of Sienna Senior Living's quality and risk management
Education & Experience:
- Registered Nurse with membership with the College of Nurses of Ontario.
- Minimum 5 years' experience in Long-term Care or Hospital setting
- Expertise in Clinical Quality Assurance
- Experience in successful Accreditation with CARF/Accreditation Canada
- Proficient with Word, Excel, PowerPoint, Outlook and Power BI
- Experience or training in PDCA/PDSA improvement cycle
