Join our team as a Licensed Practical Nurse (LPN)
The Licensed Practical Nurse (LPN) is one of a team of LPNs who are responsible for providing nursing care within their scope of practice to participants within the Community Living Support programs of the agency. Reporting to the Manager of Clinical and Community Services, the LPN acts as a liaison between participants and the person-centered support staff, promotes health and healing by functioning as a member of the care team to provide optimum care for individuals with complex medical health needs. As part of an interdisciplinary team the LPN carry out their responsibilities in accordance with statutory and regulatory standards, agency policy and within the established LPN scope of practice.
Qualifications
Education and Professional Certification
- Graduate of Practical Nursing Diploma. Licensed with the College of Licensed Practical Nurses of Saskatchewan (CLPNS).
- Licensed Practical Nurse registered in the Province of Saskatchewan
- First Aid CPR BLS/HCP
Experience
- Minimum one (1) year of nursing experience as a Licensed Practical Nurse.
- Experience working with individuals with complex needs, behavioural issues, developmental disabilities, mental health disorders, and/or other related issues is an asset.
- Ability to provide services in home settings.
Skills and Characteristics
- Knowledge and ability to apply the nursing practices and processes within the LPN scope of practice.
- Experience with care and support of vulnerable persons with complex needs, behavioural issues, developmental disabilities, addictions, mental health disorders, and/or other related issues.
- Ability to assess and facilitate learning needs of the participant as pertaining to the applicable disease, treatment and or proactive health care need.
- Strong interpersonal skills with the ability to form strong relationships and professional working alliances.
- Ability to display empathy, patience, understanding, and a person-centred approach in day-to-day interactions.
- Ability to monitor and evaluate own beliefs and behaviors with regard to prejudice and personal biases and change behavior as needed.
- Ability to remain organized and pay attention to detail in order to administer the multitude of activities to ensure participants are served in a respectful, confidential and timely manner.
- Ability to independently problem-solve and implement a solution within scope of practice.
- Ability to be flexible and adapt to new situations.
- Strong written and oral communication skills.
- Effective time management and organizational skills with the ability to prioritize.
- Computer literate, including the ability to operate a variety of desktop applications.

- Knowledge of confidentiality protocols and demonstrated ability to apply such.

Duties and Responsibilities
Services
- Provide LPN services, within defined scope of practice, to participants based on participants assessment and care planning procedures.
- Acts as the communication and coordinating link for participants consulting with a variety of practitioners while collaborating and functioning as part of the program team.
- Co-ordinate individual appointments and referrals.
- Ability to communicate effectively through both written and verbal, nurturing excellent human relations with individuals, families, and other team members.
- Communicates with other members of the team to provide continuity of participant care and promote collaborative efforts directed towards quality participant care. Admitting new participants, recording their medical history, and monitoring their care.
- Provides support, information, psychoeducation, and follow-up to individuals.
- Ability to work with minimal supervision and to evaluate and meet individual care needs, demonstrating initiative and problem-solving skills.
- Safeguards the trust of individuals that information learned in the context of a professional relationship is shared outside the care team only with individual's written permission or legal requirements.
- Provide oversight and influence related to the therapeutic milieu of the program, providing caregiver training or education sessions when required.
- Actively participate with team members in the assessment, planning and evaluation of the individual and their care.
- Monitor individual progress, evaluate and document wellbeing.
- Participates as a member of the multi-disciplinary team in the development of, and implementation of, a person-centred care plan for the individual.
- Provide safety and health education to individuals and their families.
- Perform other related duties as assigned.
Care
- Perform nursing interventions such as basic bedside care, monitoring and assessment, patient education, documentation, medication administration, taking vital signs, applying aseptic techniques including medical nutrition administration via G-tube, dressings, administering injections of medications, tracheostomy care, ensuring infection control, monitoring nutritional intake and conducting specimen collection, equipment care.
- Help participants with personal care, mobility needs, nutritional and dietary care
- Implement OT/PT/SLP/Dietary and other therapeutic recommendations along with caregivers
- Attend medical or other therapy appointments and complete necessary follow up.
- Deliver care according to the College of Licensed Practical Nurses of Saskatchewan.
- Provide ongoing health teaching to individuals in relation to common health problems and the individual maintaining a healthy lifestyle. Responsible for actioning on any emergency situations requiring immediate attention, should they arise.
- Share specialized knowledge with the person-centred support team and provide professional consultation and training to assist with the professional development of the team members.
- Provide support and health consultation services as required to the multi-disciplinary team.
Management of Resources
- Maintains the participant's file with proper documentation and safe practices related to medication administration and other care related processes.
- Provides individual and family teaching based on needs assessment.
- Maintaining current progress records on individual according to legal and ethical standards.
- Coordinate and collaborate with community resources when required.
- Provide input into budgets for special needs, events, or education sessions.
Safety
- Maintain, promote, and reinforce safe work habits, practices, and procedures.
- Adhere to written or verbal agency and government regulations, policies, practices, and instructions.
Additional Requirements
The incumbent is responsible for completing all mandatory training requirements for the position as per agency policy and guidelines. The incumbent must be registered with an accredited professional association or body.
The wage range for this position is : $34.40 to $44.46 per hour depending on experience.
