Job Summary
We are seeking a dedicated and experienced Primary Care Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive patient care, focusing on health promotion, disease prevention, and the management of chronic conditions. Duties
- Meet to Government of Alberta Guidelines of reaching at least 900 patient panel, providing after-hours access on weekends, evenings or holidays. (Full-time or Part-time week hours available) and complete the Expression of Interest Form:
- Conduct thorough physical examinations and patient assessments to determine health status and needs.
- Develop and implement individualized care plans based on clinical findings and patient preferences.
- Provide acute and chronic disease management, including medication administration and monitoring.
- Perform diagnostic evaluations, including ordering and interpreting laboratory tests and imaging studies.
- Administer treatments such as IV infusions, botulinum toxin treatments, dermal fillers, and esthetic laser treatments as needed.
- Manage patient care in various settings including outpatient clinics, urgent care facilities, nursing homes, and assisted living environments.
- Collaborate with other healthcare professionals to coordinate patient care across different services including geriatrics, pediatrics, internal medicine, and behavioral health.
- Maintain accurate medical documentation in compliance with HIPAA regulations and utilize EMR/EHR systems effectively (Healthquest).
- Educate patients on health maintenance, disease prevention strategies, and lifestyle modifications.
- Participate in clinical research initiatives to enhance patient outcomes and advance medical knowledge.
Qualifications
- Current certification as a Nurse Practitioner with a valid state license.
- Experience in primary care settings with a strong emphasis on patient-centered care.
- Proficiency in essential clinical skills including suturing, phlebotomy, venipuncture, catheterization, airway management, and emergency procedures.
- Familiarity with coding systems (ICD-9/ICD-10/CPT) for accurate medical documentation.
- Previous experience in critical care environments (ICU/PICU) or emergency medicine is preferred.
- Knowledge of geriatrics, pediatrics, infectious disease care, pain management, and behavioral health is advantageous.
- Strong communication skills with the ability to work effectively with diverse populations including individuals with developmental disabilities or dementia.
- Experience with telehealth platforms is a plus.
- Commitment to ongoing professional development through continuing education opportunities.
