Social Worker

The Social Worker provides case management, assessment, consultation, intervention and brief counselling to a broad range of patients and their families for Kings County Memorial Hospital and Souris Hospital. This position reports to the Director of Clinical Services East; with support from the Social Work Department Manager at the Queen Elizabeth Hospital when needed.
Duties will include:
- completing CSW assessments, consultation, analysis, treatment and developing case plans;
- coordinate information and services, make targeted referrals, collaborate with other community services, professionals and programs and develop treatment plans for patients and their families;
- responding to referrals from doctors, nurses, other health care team members, patients and their families on situations requiring extensive assessment of the immediate and long term needs of the patient and family;
- act as case manager to coordinate patient services between the members of the health care team and community agencies programs and professionals;
- provide stabilizing interventions for individuals and families in crisis;
- provide direction to physicians /nursing about Child Protection, Adult Protection and Public Trustee matters and Provincial Guardianship;
- provide direct intervention in mediation issues where there are incidents of patient or family conflict with the hospital/staff;
- provide assistance in the management of bed crises and work with the Nursing Units and Hospital Management of the facilities to strategize and implement solutions;
- provide an active liaison function with community based social services agencies and groups in developing case plans for patients;
- provide professional consultation and periodic in-service training to members of the health care team as appropriate in the general hospital and community;
- be a responsible advocate for patients and families within the health care system, balancing the rights and needs of the person with the limitations of the system;
- attend Rounds as required at both facilities;
- represent the East at provincial or regional meetings as required;
- provide supervision to graduate level students as required;
- other duties as required.
Minimum Qualifications:
- Applicants must have a Master's degree in Social Work with clinical focus from an accredited school with considerable experience.
- Must be registered with the PEI Social Work Registration Board and be in good standing or be eligible for registration in good standing.
- Must have considerable experience practicing as a Clinical Social Worker
- Working knowledge of Mental Health Act, Child Protection Act, Adult protection Act and other relevant Federal and Provincial statutes and acts, the Diagnostic and Statistical Manual for Mental Disorders.

- Demonstrated ability to work independently within professional expertise and apply sound clinical decision making;
- Demonstrated ability to work cooperatively and collaboratively in an interdisciplinary team environment;
- Have or committed to acquire training (designated population/program/service) necessary to perform role;
- Demonstrated leadership, interpersonal, facilitation, and collaboration skills and commitment to continuous professional development;
- Ability to develop and lead education sessions for patients, families, staff, physicians and management;
- Strong communication skills, both written and verbal and effective time management and organizational skills;
- Conflict resolution skills;
- Ability to work independently and as part of a team, while respecting professional boundaries, cultural differences, team decisions, and respect for diversity;
- Ability to exercise diplomacy, tact and judgement in collaborating with patient's, families, and staff in difficult and complex situations;
- Proficiency working in a computerized environment;
- Previous experience in a hospital Social Work setting would be considered an asset;
- Experience and knowledge in making community placements, accessing in-home supports, or supported housing particularly for seniors; training in critical incident debriefing;
- Additional relevant education and experience will be considered an asset.

Salary Range:
$ $50.42 per hour
Bi-Weekly Hours:
75 Hours Bi-Weekly / 7.5 hr. Shifts / Days / Weekends may be required depending on service needs
