SUMMARY:

Funded by Region of Peel CARE Program, Connection, Acceptance, Resource, Empowerment (CARE) Program will increase capacity of community-based organizations to address the mental health and wellbeing needs of individuals accessing Ontario Works in Peel and other Region of Peel Human Services clients. The program will provide clients with a suite of services including Mental Health services, case management, crisis care, advocacy, and system navigation support. These services will be delivered through a trauma-informed and client-centered approach, ensuring that each client's unique needs were met.

We are seeking a Mental Health Counsellor & Navigator, who will report to the Manager - Women, Youth & Specialized Services. The incumbent will support clients participating in the Region of Peel's Ontario Works program by delivering comprehensive mental health assessments and supportive counselling, crisis response, and navigating mental health and addiction services provided by the agency and in the community.

DUTIES & RESPONSIBILITIES:

  • Complete initial engagement and pre-screening to verify eligibility (such as social assistance status, social determinants of health, mental health history and challenges) and to gain understanding on client's mental health and social support needs.
  • Develop expanded and enhanced Mental Health Toolkit with comprehensive range of mental health resources (i.e. Self-help resources and coping methods, culturally navigating mental health systems, and guide to navigating mental health systems).
  • Ensure intake and informed consent forms are available in various languages and formats to ensure accessibility for all clients, including those with disabilities or languagebarriers.
  • Conduct holistic clinical assessment of mental health symptoms, history and previous intervention of referred individuals and identify existing supports (i.e. family, caseworkers, peer networks) and provide support during crises.
  • Assess safety, psychosocial and environment evaluation to understand client's and family's life as requested by the client (i.e. housing stability, employment status and financial stressors etc.).
  • Develop comprehensive goal-oriented care plan (short-term and long-term) aligning with program's integrated service model
  • Utilize trauma-informed approach to gather information to make informed decision on the client's program eligibility, readiness to engage/benefit from services, and availability of sustainable supports.
  • Conduct ongoing monitoring, reassessment, and navigation support with regular check-ins and progress reviews.
  • Liaise with and collaborate with representatives of relevant community agencies/services to help navigate clients through community resources to develop a care plan and obtain goals.
  • Provide comprehensive information to the client on various community resources related to their care plan goals.
  • Maintain accurate and confidential records of client interactions, including the gathering of statistics and other records to be used in internal and external reports.
  • Represent the CARE program in community networking meetings and events as required
  • Maintain strict confidentiality of client information.
  • Maintain positive and clear communication with other team members to facilitate teamwork, successful target management and customer service.
  • Perform other related duties, as required and as assigned.

QUALIFICATIONS:

  • University Degree in a related discipline, such as Social Work or similar
  • Three (3) years of recent and relevant clinical work experience ideally within a non-profit or community health environment
  • Registered with the appropriate regulatory body, such as Ontario College of Social Workers and Social Service Workers, or the College of Registered Psychotherapists of Ontario
  • Experience with intake procedures, case management, and crisis intervention
  • Excellent knowledge in mental health, justice and social assistance systems, community agencies/resources, housing supports, child protection, family law, immigrant and refugee issues, and legislative issues
  • Extensive experience working within a trauma-informed anti-racist/anti-oppressive and gender-based violence framework while establishing a psychologically safe environment
  • Excellent interpersonal, communication, and relationship building skills with the ability to work and collaborate within a team environment
  • Strong computer skills (i.e., MS Office)
  • Excellent negotiation, conflict resolution, crisis intervention, mediation and active listening skills
  • Experience with various administrative tasks such as data entry, tracking, data collection, report writing, etc.
  • Ability to travel for internal and external stakeholder meetings and conferences
  • Willing to work evenings and weekends as dictated by program needs
  • Provision of a Criminal Reference Check if required for specific funder/program demands.
