LOCATION: Whitby, with responsibility throughout Durham region.

PURPOSE OF POSITION
Serving as a regional resource, the Complex Care Case Manager is responsible for providing comprehensive case management supports to individuals (children, youth, young adults) and families who receive Complex Special Needs (CSN) funded supports. Complex special needs funded supports may include: long-term in-home or in-community supports or resources, supported living placement in a regional CSN designated Transfer Payment Agency or supportive living placement in an Outside Placement Resource (OPR).
This is a specialized case management function that provides for: i) monitoring of the implementation of the CSN funded service plan; ii) ongoing evaluation to ensure that the plan continues to be relevant and in the child's or youth's best interest; iii) accountability that CSN funds are being spent appropriately and, iv) referral to and support to access a range of required services.
STATEMENT OF MAJOR RESPONSIBILITIES
- To assist with the development, implementation and monitoring of the regionally approved integrated service plan (plan). In so doing, the Complex Care case manager:
- Meets with all relevant parties (including the individual, family, community providers) to ensure that everyone is in agreement with the plan and to gather information and begin to plan for implementation
- Researches the availability and suitability of supportive living placement resources if these are specified in the plan
- Researches the availability and suitability of in-home or in-community resources if these are specified in the plan
- Develops a budget for implementation of the plan
- Works with the family and community and/or supportive living resources to plan for the transition. Provides supports (including counselling) as feasible and appropriate to the family and child to help them cope with the transition
- Works to ensure that all aspects of the plan are implemented as intended and in as seamless a manner as possible
- To ensure that the plan meets (and continues over time to meet) the needs of the child or youth and family, the complex Care case manager:
- Formulates a current understanding of the individual's needs, respecting values, preferences, beliefs, heritage and identity on an ongoing basis
- Arranges for and/or facilitates the provision of specialized assessments as needed
- Participates in the development and monitoring of the plan of care in residential settings in accordance with CYFSA/SIPDDA requirements
- Refers and assists in accessing required services
- Provides social advocacy support
- Monitors delivery of contracted services identified in the plan qualitatively and monetarily
- Evaluates the plan to ensure ongoing appropriateness to meet individuals
- Initiates planning for significant transitions and works to make these transitions as seamless as possible
- To engage with the family of the child youth or young adult in order to support and facilitate their ongoing involvement in their child's life (e.g., through counselling, working with supportive living resources or the use of quasi-contracting).
- To maintain an organized and accurate paper/electronic clinical record that demonstrates the provision of services, establishes accountability, and enable the evaluation of service quality for all persons served that includes:
- Recording and reporting client information necessary for effective case management and preparing progress reports
- Collecting and reporting service delivery data as required
- Adhering to all Regional Complex Care service/agency standards and guidelines and
- To promote the Regional Complex Care Service through presentations, literature and inter-agency functions.

- To participate in professional development in consultation with manager, and demonstrates initiative in learning and developing new techniques of service
- To practice effective communication with co-workers, consulting and sharing resource information and providing case coverage as required.

QUALIFICATIONS
- Education and Experience Level:
- Post secondary degree at a university level in one of the social or behavioural disciplines with directly related experience (3 to 5 years) in working with children/youth and adults with multiple and complex areas of exceptionality (developmental disability, emotional/psychiatric needs, medically fragile, acquired brain injury) and their families
- Required Knowledge and Skills:
- Knowledge of normal and abnormal
- Knowledge of family systems theory and an understanding of the family development and functioning.
- Ability to develop trusting, co-operative relationships with clients, families, service providers, and other stakeholders.
- Excellent communication and negotiating skills and judgment and ability to manage
- Knowledge of community resources for youth and adults with special needs and relevant
- Ability to support self-advocacy and empowerment for
- Sensitivity, understanding and ability to work with a variety of value systems, beliefs and
- Working knowledge of word processing and data
- A working understanding of French Language is
- Capacity to work
- Aware of Health & Safety Rules &
- Valid driver's license and access to a reliable vehicle
Hourly Rate Range: $30.38-$34.22 per hour plus, a generous employee Benefit package, including Wellness Program, Employee Assistance Program, Pension Plan & Health and Dental Plan.
