Position Summary:
The Team Leader is responsible for ensuring Individuals Accessing Services (IASs) receive the support they need to achieve their goals and lead fulfilling lives. This involves effectively managing designated programs with a caseload of IASs that benefit from community engagement activities as well as residential services. The Team Leader ensures that the day-to-day needs of IASs are being met through ongoing liaison with workers (employees and contractors) and all stakeholders. This position ensures appropriate steps are taken if any serious issues or concerns emerge. The Team Leader plays a lead role in the development of client support plans (ISPs, PPPs, and RPs) and liaises with workers to update and maintain these documents and other required documentation through ShareVision.
The Team Leader ensures that workers under their supervision are equipped and empowered to successfully execute their roles to provide the best services possible. This position creates a culture of engagement, recognition, accountability, and high performance. This is achieved through ongoing coaching, mentoring, and leading by example so workers have the confidence and support to make decisions and solve problems within their delegated authority. The Team Leader must follow all Vantage policies and procedures, health and safety protocols, legislative protocols, strive to meet CET standards at all times, and ensure that their teams do the same.
Accountabilities:
1. Ensure that the day-to-day needs of IASs are being met and that Vantage policies and procedures are being followed
- Liaise with workers, guardians, trustees, families, IAS networks, and other community support services and resources
- Liaise with guardians and trustees to ensure quality of services, ongoing informed consent, and any serious IAS concerns
- Provide oversight to contractors through ongoing communication, conducting regular home visits and relevant documentation are noted and uploaded
- Ensure that any incidents are reported and that the appropriate process is followed for both critical and non-critical incidents
- Provide all relevant information and support to the Director, Projects & Risk Management (or designate) to complete any abuse investigations
- Ensure the development of emergency plans for IASs, all safety measures are in place within homes, and all workers are trained in health and safety and emergency procedures
- Provide on-call support as per the rotational schedule
- Review and approve reconciliation of client funds
- Create and maintain quarterly/monthly/weekly/daily schedules as per program requirements
- Work with Human Resources in the hiring and onboarding of new workers
- May need to step in to do frontline work when there are significant staffing issues
2. Ensure that IAS support plans are completed so that they can achieve their goals, and the best approaches are used by all workers
- Lead and assess the research, development, and implementation of programs to meet IAS outcomes
- Take a lead role in the development, monitoring, and updating of Individual Support Plans (ISPs), Planned Positive Practices (PPPs) and Restrictive Procedures (RPs)
- Gather consents and other required documents from stakeholders, guardians, IAS, and other service providers
- Liaise with workers to complete and update support plans and ensure that outcomes tracking is completed throughout the year on ShareVision. This includes outcome updates, changes to IAS information, and all relevant information is up to date for each IAS
- Ensure that other required documentation is being completed appropriately in a timely manner
3. Effectively supervise workers so they are empowered and equipped to provide the best services possible
- Ensure 1:1s and informal check-ins are done on a regular basis throughout the performance management cycle
- Ensure annual performance goals are set, ongoing feedback is provided, and performance evaluations are done in a timely manner
- Support the growth and development of workers through ongoing coaching, mentoring, and supporting job-related formal and informal training
- Foster a culture of recognition and appreciation
- Empower workers to make decisions confidently based on the position's delegated authority
- Follow, implement, and work with the Human Resources Department to ensure employment standards, human rights, and common law obligations are met
- Ensure worker qualifications are up to date
- Authorize leave requests and timesheets
- Facilitate team/support meetings on a regular basis
4. Manage all administrative requirements of contractors
- Approve and forward all monthly invoices from contractors and respite workers to Accounting Team
- Complete compensation letters for respite workers
- Ensure that all contracts and funding letters are completed, and renewals are done in a timely manner
- Ensure that all mandatory training is up to date and submitted
- Ensure that all contract obligations are being met and if there any ongoing concerns that the proper process/recourse is implemented
- Manage the recruitment of contractors to ensure appropriate matches are made based on specific IAS criteria
- Ensure that contractors have the appropriate training needed to fulfill contract requirements, which often requires one-on-one coaching to contractors that are not digitally strong or able to complete documentation appropriately

Qualifications/Experience
- Two-year diploma in Human Services Field; degree in a related field preferred
- 3-5 years of experience in a relevant field
- 2-3 years of supervisory or augmented leadership experience
- Mandatory training (First Aid & CPR, The Mandt System, ASIST, Abuse Protocol, Mental Health First Aid, Med Assist, Universal Precautions, and PPP & RP) and other individual specific training (training and recertification opportunities are provided by Vantage)
- Must have a clear Vulnerable Sector Check (applied within the last 6 months)
- Proficient with Microsoft 365
- Flexibility with schedule and work time is needed due to weekend on-call rotation, meetings with contractors, and urgent needs that may arise
- Possession of a reliable car with personal liability insurance, providing a minimum coverage of two million ($2,000,000.00)
- Possession of a Class 5 - Non GDL Driver's License with a clean drivers abstract and no restrictions
