Community Wellness Supervisor-Aftercare

Regular Full Time

TFN is seeking a Community Wellness Supervisor to oversee daily operations at the Men's Healing House, lead frontline staff, and provide post-vention support for members transitioning back into the community. Reporting to the Manager of Family Services, this role works alongside a second team focused on prevention to ensure continuity of care for members experiencing life's challenges, while supporting culturally safe, high-quality services. The Supervisor works closely with TFN members, internal staff and external professionals to ensure community members receive timely, accessible, and wholesome care.

Key Responsibilities

  1. Post-Care Coordination & Case Management

  2. Develop individualized reintegration and wellness plans for TFN members returning from treatment, corrections, or other care settings.

  3. Provide ongoing case management, monitoring progress, and adjusting care plans as needed.

  4. Ensure all post-care services (housing, counselling, employment, cultural supports) are coordinated smoothly.

  5. Culturally Grounded Wellness Support

  6. Embed TFN cultural teachings, values, and practices into wellness planning and service delivery.

  7. Facilitate access to Elders, knowledge-keepers, cultural programs, and land-based healing when appropriate.

  8. Promote culturally safe and trauma-informed approaches to care.

  9. Navigation of Health & Social Services

  10. Connect members with mental health, addictions, medical, and social support services.

  11. Support clients in navigating systems such as provincial health, income assistance, housing programs, or justice-related services.

  12. Advocate for client needs with external agencies.

  13. Monitoring Safety & Stabilization

  14. Support members in maintaining sobriety, healing, and life stability after returning to the community.

  15. Identify early signs of relapse, crisis, or safety concerns and intervene appropriately.

  16. Implement crisis response plans and work with emergency services when required.

  17. Community Reintegration Support

  18. Assist clients with rebuilding relationships with family, community, and cultural groups.

  19. Support re-engagement in education, employment, training, or community roles.

  20. Foster independence and self-determination in members' wellness journeys.

  21. Program Development & Evaluation

  22. Help design, improve, and evaluate post-care and reintegration programs based on community needs.

  23. Collect data and track outcomes to ensure services remain effective and culturally aligned.

  24. Contribute to policy updates, best practices, and service quality improvements.

  25. Interdisciplinary Collaboration

  26. Work closely with TFN Member services, housing, education, and administrative departments.

  27. Liaise with external partners such as treatment centers, probation officers, clinicians, and cultural service providers.

  28. Facilitate case conferences and interagency communication.

  29. Community Outreach & Engagement

  30. Build trusting relationships with members through respectful, strength-based engagement.

  31. Provide outreach to members.

  32. Promote community awareness about wellness resources and harm-reduction supports.

  33. Documentation & Reporting

  34. Maintain accurate, confidential case files compliant with privacy and TFN policies.

  35. Prepare progress reports, incident reports, and program summaries for leadership.

  36. Track service delivery and follow-up outcomes.

  37. Leadership & Team Support

  38. Mentor or guide frontline wellness workers, peer mentors, and outreach staff.

  39. Model trauma-informed, culturally safe, and ethical practice.

  40. Participate in training, professional development, and team meetings.

Qualifications and Experience:

  • Diploma or degree in social services, or a related field, lived experience is an asset

  • 2+ years of experience in community or working within recovery-based settings

  • Strong understanding of Individualized care planning and working in a group setting

  • Experience supervising staff, coordinating programs, and managing day-to-day operations

  • Excellent communication, organizational, and leadership skills

  • Knowledge of Indigenous care systems, modern and traditional approaches to healing and restoration, and ability to understand the complexities and challenges navigating services

  • Valid Class 5 Driver's License and access to reliable transportation

Working Conditions:

  • On-site role based at TFN's Men's House

  • Standard hours Monday-Friday, 8:30 AM-4:30 PM, with occasional evening or weekend work based on community needs

  • Involves working in fast-paced, client-centered environments that may include urgent or sensitive matters

Wages:

Pay scale for this position is $40.97 to $43.38 per hour (e.g. $79, 891 to $84, 591 per annum) with excellent benefits including enrolment in the BC Municipal Pension Plan. Placement within the pay scale is based on a review of skills, experience & internal equity.
