This can be completed through the pre-screening questions or under the RSS option in your applicant account under Basic Information.Position Summary:The Personal Support Worker (PSW) is part of an interdisciplinary team providing direct care, intervention, and support to older adults living in Fairmount Home (Fairmount); transitioning into or between healthcare and/or long term care settings within the geographical boundaries of the South East Local Health Integration Network (SELHIN). The PSW will work with residents who are demonstrating responsive behaviours associated with the various types of dementia, delirium, mental health, addictions, or other neurological conditions.The Behavioural Support PSW provides assistance with personal care and activities of daily living for program residents. Working in collaboration with the in-home Behavioural Support liaison, Geriatric Psychiatry team, Medical team, Registered Nurse, Registered Practical Nurse, and the PSW's, the PSW will enhance the quality of life for residents by participating in the assessment of responsive behaviours; providing direct care and support; identifying triggers and successful strategies for responsive behaviours assisting and modeling of these strategies in collaboration with Long-Term Care Home (L TCH) staff. The PSW will carry out the duties in alignment to the organization's mission, vision and values, and work to further its priorities.Education and Experience:Post-secondary Personal Support Worker certificate from a recognized Community College and/or an accredited program as legislated in the LTCHA.Equivalent combination of relevant education and experience as identified in the LTCHA may be considered including Graduate Nursing Assistant, Registered Practical Nurse, Graduated Nurse, Registered Nurse.Recent and related experience with residents in a long term care setting or other health-care setting.Experience with quality improvement processes an asset.Gentle Persuasion approach training (or any related responsive behaviours education) preferred.Further details can be found under the "Career Opportunities" section of the County of Frontenac website.
