Mobile Wellness Intake/Care Coordinator

Under the direction, accountability, and guidance of the Mobile Wellness Team Lead, the Mobile Wellness Intake/Care Coordinator serves as the primary point of contact for Mobile Wellness services. This position facilitates triage, screening, and intake of individuals seeking services, determining the most appropriate type and level of care required.The Intake/Care Coordinator maintains a thorough understanding of available community services and resources and demonstrates strong system navigation skills. Case consultation, case conferencing, and advocacy with community partners and service providers are integral components of this role.Working within an interdisciplinary team and guided by the Seven Grandfather Teachings and the Woven Blanket Model of Care, the Coordinator collaborates and consults with internal and external partners to facilitate referrals, coordinate care, and ensure service continuity. The Mobile Wellness Program provides specialized services to individuals experiencing serious mental illness; therefore, the coordinator must possess an in-depth understanding of treatment pathways, crisis intervention, and recovery-oriented approaches.
Key ResponsibilitiesService-Specific Responsibilities
Intake
- Conduct comprehensive intake and assessments for individuals seeking Mobile Wellness services, including suicide risk assessments and safety planning as required
- Triage and assign clients to the most appropriate Mobile Wellness team member and/or facilitate internal referrals to other providers within the Mental Health and Addictions team
- Collaborate with referring agencies to gather additional information and provide updates on referral status
- Complete administrative tasks associated with intake, including sending waitlist letters and maintaining accurate client files and records
- Monitor and manage the Mobile Wellness program waitlist to ensure timely and equitable access to services
- Maintain a strong understanding of crisis management, serious mental illness, concurrent disorders, and the impact of trauma on mental health and addictions
- Participate in regular group consultation and case discussions with the Mental Health and Addictions team at Noojmowin Teg Health Centre
Navigation
- Coordinate and facilitate access to traditional healing, land-based, and other culturally relevant programming in collaboration with Elders, Knowledge Keepers, and Traditional Healers
- Support clients in accessing external health and social service programs, ensuring smooth transitions and ongoing communication between service providers
- Liaise and advocate with community agencies, including hospitals, shelters, and partner organizations such as Manitoulin Health Centre, to support coordinated care
- Maintain a comprehensive understanding of local resources and service pathways across Manitoulin Island
Community and Program Development
- Participate in the promotion of Mobile Wellness services through outreach, education, and community engagement activities
- Attend and contribute to community and organizational committees, working groups, and program planning initiatives as directed
- Identify service gaps, trends, and barriers, and share recommendations with the Mobile Wellness Team Lead for continuous quality improvement
- Complete all charting, documentation, and data entry within the Electronic Medical Record (EMR) system accurately and in a timely manner
- Compile and submit reports, statistics, and quality indicators required by funders and the organization
- Ensure strict adherence to privacy, confidentiality, and PHIPA/FIPPA standards
- Adhere to organizational policies, procedures, and professional standards of practice
- Maintain active membership and compliance with the standards of the applicable regulatory college
- Demonstrate adherence to traditional protocols and cultural sensitivity when providing services within First Nation communities
- Complete all required leave documentation (e.g., annual leave, professional development, certified or uncertified sick leave, overtime, etc.) in a timely manner
- Participate in professional development, certification, and training as required by Noojmowin Teg, funders, or legislation
- Attend staff meetings, strategic planning sessions, wellness activities, annual general meetings, and other required organizational events
- Perform other duties as assigned
Mandatory
- Bachelor of Social Work degree; a related health or social services degree with relevant experience may be considered
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers or an equivalent professional regulatory college
- Minimum of three (3) years of experience providing clinical or community-based services within a mental health and/or addictions setting
- Demonstrated experience in case management, crisis response, and service navigation
- Valid Class G driver's licence, access to a reliable vehicle, and a clear driver's abstract
- Clear Vulnerable Sector Criminal Record Check
Preferred
- Experience working with First Nation communities and/or Indigenous organizations
- Familiarity with electronic medical record systems and data collection processes
- Knowledge of community resources and service providers within the Manitoulin Island region
- Ability to speak or understand Anishinaabemowin considered an asset
- Strong clinical judgment and assessment skills with the ability to triage and manage complex cases
- Demonstrated knowledge of serious mental illness, concurrent disorders, and trauma-informed care
- Understanding of Anishinaabe culture, traditional wellness, and Indigenous models of health and healing
- Excellent interpersonal, communication, and advocacy skills
- Ability to work collaboratively as part of a multidisciplinary team and maintain positive relationships with community partners
- High attention to detail, organizational ability, and strong documentation skills
- Proficiency in Microsoft Office Suite and comfort working within electronic documentation systems
- Ability to work independently with minimal supervision, managing competing priorities in a fast-paced environment
- Services are delivered across seven First Nation communities and other regional sites throughout Manitoulin Island
- Frequent travel and occasional evening or weekend work may be required
- Combination of office-based, community-based, and mobile/outreach service environments
- Exposure to emotionally distressing situations may occur; self-care practices and debriefing supports are available
