Opportunity
The Community Outreach Coordination & Integration Program delivers an integrated coordinated response to vulnerable people in Chilliwack by providing a service delivery model for at-risk, high need individuals who require an intervention-based non-emergency response, often directly related to substance use, mental health, and homelessness.
The Community Integration Outreach Workers (CIOWs) will be part of a larger team of two fulltime Outreach staff with backgrounds in social work, housing, substance use and mental health. The CIOWs will provide front-line intervention and assessment of needs and support, through case management, coordination of the services required, and the follow-through for ensuring the best success of these coordinated efforts.
Responsibilities
- Provide culturally competent and trauma-informed outreach support and case management services to the most vulnerable population in Chilliwack
- Provide support during 'walk-abouts' with the RCMP, Bylaw and Security officers as planned/requested through the shift.
- Supportive conversations and on-site needs assessments with high-risk individuals
- Safe transportation to hospital or shelter
- Provision of basic needs and harm reduction supplies, including naloxone
- Case Management of complex individuals to coordinate already-existing resources and provide referral and advocacy for appropriate new supports.
- "On-Call" for RCMP, bylaw and security to provide immediate response to individuals in need during the operating hours
- Provide a safe warm handoff to appropriate referred supports
- Provide coordination and follow-through with the identified team to establish the most effective wrap-around support
- Keep daily case notes for clients who are case managed through our internal case management system.
- Keep a daily log of activities and collection of stats for reporting purposes and communication with the rest of the team.
Qualifications
- B.A. in a related field plus one-year related experience, or a combination of related work experience/education with vulnerable adults
- Criminal record check including the vulnerable sector
- Standard First Aid certificate and CPR
- Valid BC driver's license; Driver's abstract must be submitted.
