Program Coordinator, Community Mental Health

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in the provision of person-centered and recovery oriented care the Program Coordinator is responsible for overseeing and coordinating the delivery of care in the designated service area by planning, developing, implementing, and evaluating its effectiveness, efficiency and accessibility. Acts as a senior clinician in the provision of case consultation and clinical review and may provide direct case management services to community clients. Monitors, reviews, and revises policies, procedures and standards, supervises designated staff, orientates, trains and evaluates staff performance and plans for ongoing staff training needs. Collects and analyzes statistics, participates in the budget process, recruitment and selection of staff. Consults and liaises with other MH&SUS staff, other departments and community services.
- Coordinates and supervises the provision of care in the designated service area and is accountable for its overall effectiveness. Coordinates patient/client care assignments, provides information and consultation to program staff and students; reviews, problem solves and provides guidance in the management of patient/client/family care need
- sEstablishes clinical priorities, assigns and adjusts work assignments, ensures that necessary resources are available, promotes a positive team environment
- .Participates in recruitment, interviewing and selection of staff; provides leadership by developing, implementing and evaluating staff orientations, in-service education and placement of students for clinical practice experience
- .Assesses, develops and evaluates the skills and performance of staff by providing individual coaching and instruction to staff as appropriate; conducts performance evaluations and ensures the maintenance of standards, protocols and procedures
- .Provides clinical expertise, consultation, role modeling and leadership in evidence-based practice to staff and/or patients/clients by reviewing literature, consulting with experts, collaborating with members of the interprofessional team, evaluating clinical practice and recommending changes to existing standards
- .Ensures the effective and efficient patient/client flow and service utilization
- .Develops and implements quality improvement processes by methods such as engaging in programs/practice evaluation, and identifying areas for improved efficiency and effectiveness in service delivery. Participates in quality assurance audits, facility reviews and facility or staff investigations as required. Assists in resolving client and family complaints
- .Accountable for the optimal use of patient/client care resources; identifies opportunities for improved utilization of resources, provides input into budget/staff allocations, assists team members in workload decision-making, evaluates and identifies required patient/client care products and/or equipment, and recommends changes to existing administrative processes. Keeps manager informed and apprised of all recommendations or decisions
- .May be required to assist Manager in negotiating and writing contracts. Acts as a resource to the funded agencies and performs regular contract evaluations
- .Maintains, reviews and analyzes program specific data; identifies trends and issues in the provision of mental health & substance use services. Proposes, develops and implements modifications and initiatives to meet emerging MH&SUS needs
- .Oversees the development of policies, procedures, standards of care and is accountable for their effectiveness; makes recommendations to the Manager on long term planning; implements processes and procedures and evaluates results
- .Develops and maintains effective and efficient communication links with other health service providers, community agencies and other external partners
- .Participates in research and special projects by collaborating with members of the interprofessional team promoting staff awareness and involvement in research activities; identifies practices/issues that require research; collects and interprets data and provides input for further analysis
- .Participates in local, regional and external committees and working groups as assigned and provides input into operational issues
- .May be required to provide comprehensive clinical case management services to a limited number of community clients. Documents assessments, care plans, observations, interventions, outcomes/evaluations and referrals according to practice standards and Fraser Health procedures
- .Performs other related duties as assigned
