Job Description:  As a member of an interdisciplinary team and under the supervision of the Manager, the incumbent will be responsible for creating a therapeutic environment in which you will deliver a high standard of care to patients who are diagnosed with substance dependence and Mental Health concerns. The community Provincial Addictions Nurse Coordinator will provide intensive case management, physical assessment and substance withdrawal screening in order to recommend appropriate treatment options and develop a care plan. Additionally, mental health and suicide risk assessments will be required and safety planning for clients will be completed. As an Addictions nurse coordinator, naloxone training and formal community education will be required. The Addiction Nurse Coordinator will provide formal pharmacological education and addiction medicine education to Pharmacies, Labs, Emergency Departments, Physicians and SHA colleagues or community partners assisting with continuum of care for clients. Responsibilities include providing individual counselling which may include brief interventions, ongoing medication management counselling and crisis counselling. The Addiction Nurse Coordinator will assist clients with complex presentations, independently assess and determine and coordinate treatment while utilizing their clinical skills and expertise to direct unregulated health care workers and/or care providers. The incumbent will provide medication education to patients and assist Physicians in determining addiction medication options based on the Nurses assessment. The incumbent will perform other nursing duties as required

Education

  • Baccalaureate Degree or Diploma in Nursing or Psychiatric Nursing.

Experience 

  • Minimum 2 years of experience in a Mental Health and Addiction setting is required. Community Mental Health and Addiction experienced is preferred.
  • Demonstrated clinical expertise in assessment, diagnosis and treatment of major mental health disorders, withdrawal, addiction medicine and substance dependency issues.

Knowledge, Skills & Abilities

  • Excellent knowledge and skills related to Nursing process and interventions in relation to mental health and addiction of the individual, including the ability to utilize a variety of assessment tools, ie mental status exam, withdrawal assessments (ciwa, Owa/cows, Baws)
  •  Knowledge of evidenced based practices in treating concurrent, substance dependent and mental health patients.
  •  The ability to make sound clinical decisions with limited immediate supervision.
  •  Initiative and professional judgement in planning appropriate interventions within the multidisciplinary team.
  •  Ability to plan, organize, and coordinate efforts with multidisciplinary team members and communicate effectively.
  • Act as a mentor to community partners and colleagues throughout the SHA with regards to professional standards and accountabilities.
  • Team building skills, leadership, assessment and goal planning skills.
  •  This position has no subordinates but provide clinical supervision amongst the team to ensure safe care and serves as a resource to other professionals, ie nursing students, residents, unregulated health care workers, pharmacists, acute care staff.
  •  Excellent knowledge of mood altering medications and how these affect clients.
  •  Demonstrated competency in psychiatric mental health assessment.
  •  A demonstrable knowledge of substance dependency/co-dependency and the effects on the family unit.
  •  Ability to accurately assess substance dependency/dual diagnosis/co-dependency and make appropriate recovery plans and referrals.
  •  Knowledge of Indigenous culture and practices.
  •  Knowledge of group dynamics.
  •  Demonstrates knowledge of a Holistic Addiction Treatment Model and the Transtheoretical Model of Change.
  •  Ability to develop and present workshops and formal education to clients and the community.
  •  Ability to communicate effectively orally and in writing.
  •  Knowledge of community resources in the addictions field.
  •  Ability to work independently with minimal supervision, prioritizing workload demands while collaboratively addressing clients overall treatment goals.
  •  Participate in program development, committee work and program evaluation process as required. Nurses will provide valuable insight on effectiveness of programming and participate in improvement work.
  • Knowledge of best practices in harm reduction strategies.

Other

  • Scheduled hours of work will be reduced during the rotation stipulated when a statutory holiday(s) is recognized during that period
  • Current registration with the Registration Psychiatric Nursing Association of Saskatchewan (RPNAS) or College of Registered Nurses of Saskatchewan (CRNS) is required.
  •  Current PART and CPR certification required.
  • Valid driver's license and use of a vehicle for work as travel may be required.
