Location: South Surrey, White Rock, and community outreach

Hours: 35 per week [Monday 11:30 – 7:00pm, T-Th 9:00 – 4:30pm, F 7:00 – 2:30pm (on call)]

Salary: $27.81 per hour

Start Date: ASAP

We are looking for a System Navigator to join our System Navigation team This individual must be authentic, compassionate, and knowledgeable about the social services system. They must have a strong understanding and full acceptance of harm reduction philosophy. This position requires current Standard First Aid Certification and a Class 4 Driver's License. The successful candidate can work towards acquiring Class 4 Driver's License within the probation period.

Description of Position:

Reporting to: The Manager of Overdose Prevention, Peer Support, and System Navigation Services

General Responsibilities:

  • Outreach to various areas of the South Surrey/White Rock community to locate people who are experiencing multiple barriers to wellness, such as substance use, mental health challenges and homelessness.
  • Engages with clients in a trauma-informed manner to support them in developing physical, social, emotional and life skills.
  • Shares lived-experience perspectives to build rapport and trust and modelling harm reduction practices, self-determination and wellness strategies.
  • Supports clients in identifying personal goals for increased well-being.
  • Assists clients to find information about and gain access to local resources; supports clients to navigate and overcome barriers to receiving care; transports clients to resources and services while ensuring the security of unhoused clients' belongings.
  • Works collaboratively with clients as they set and work on their self-identified goals; supports clients to break down goals into manageable steps, and assists them to achieve graduated success in all areas of their lives.
  • Provides opportunities for clients to increase their engagement in community integration activities; supports clients to develop meaningful connections with a wide range of local resources and supports.
  • Responsible for case management, case note recording and monitoring goal attainment.

Program-specific Responsibilities:

  • Provide comprehensive case management to support the achievement of client-identified goals.
  • Outreach, as a team, to foster relationships with vulnerable individuals.
  • Connect with other Peer Support Network drop-in services and Overdose Prevention Services, and collaborate with other service providers.
  • Transport clients to access health care and social services, including but not limited to income assistance, substance use services, mental health supports, and Opioid Agonist Treatment.
  • Supports clients to secure resources, including but not limited to shelter or sustained housing, identification, cell phone, and food security.
  • Responds to urgent client needs as they arise.

Qualifications:

  • Experience navigating the systems of health care, mental health and substance use services and social services
  • Lived experience of mental health, substance use or housing insecurity will be considered as a valuable asset in this role
  • A certificate or a diploma in a social service-related field and experience supporting vulnerable populations is preferred.
  • Two years of recent related work experience with an emphasis on substance use and/or support to unhoused individuals
  • Thorough knowledge of harm reduction and trauma-informed practice
  • Proficiency in Motivational Interviewing or other engagement strategies is an asset
  • Effectively utilize the Office suite, including Excel, Word, Outlook and central data systems
  • Knowledge of community resources and programs
  • Ability to analyze and problem solve
  • Strong organization and prioritization skills
  • Ability to engage with diverse clients who have a wide range of needs, abilities, strengths and challenges
  • Ability to establish and maintain rapport and appropriate boundaries with clients and their families
  • Ability to model, instruct and demonstrate life skills.
  • Effectively communicate professionally, verbally and in writing in English
  • Class 4 BC Driver's License (Mandatory)
  • Standard First Aid and CPR (Mandatory)
  • Indigenous Cultural Training (Mandatory) (Training provided by Sources)
  • Naloxone and Overdose Prevention Training (Mandatory)
  • Non-violent Crisis Intervention Training (Mandatory) (Training provided by Sources)
  • Criminal Records check through the Ministry of Public Safety and Solicitor General Criminal Records Review Program
