Cranbrook Urgent and Primary Care Centre is seeking a dedicated Physiotherapist to join our collaborative inter-professional team. About the Role

Primary Care Centres and Urgent and Primary Care Centres (UPCCs) are community-based networks where healthcare professionals work together to deliver coordinated, patient-centered services. UPCCs provide same-day, non-emergency care for people needing urgent attention, as well as ongoing primary care for individuals who may not have a regular provider. They form a vital part of the region's Primary Care Network (PCN), ensuring that patients receive the right care, by the right provider, at the right time—close to home.

In this role, the Physiotherapist works as part of an inter-professional care team, collaborating with primary care network physicians, nurse practitioners, nurses, and allied health colleagues. Responsibilities include:

  • Conducting comprehensive client assessments and care planning
  • Providing individual and group therapy sessions
  • Supporting clients in ongoing self-management and monitoring
  • Working with clients of all ages, including adults, seniors, and pediatric populations
  • Supporting individuals with complex and chronic conditions
