Job Summary

We are seeking a compassionate and dedicated Registered Home Health Care Aide to provide essential support to individuals in their homes or community settings. The ideal candidate will possess a strong commitment to enhancing the quality of life for patients, particularly those with developmental disabilities, dementia, or Alzheimer's. This role involves assisting with daily living activities, ensuring patient safety, and maintaining a clean and supportive environment.

Responsibilities

  • Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation.
  • Observe and report changes in patient condition and behavior to healthcare professionals.
  • Administer medications as prescribed and maintain accurate medical documentation.
  • Provide companionship and emotional support to enhance the well-being of patients.
  • Assist with driving to and from appointments
  • Perform light housekeeping duties including cleaning, laundry, and meal preparation to ensure a safe living environment.
  • Monitor vital signs and assist with catheter care as needed.
  • Maintain compliance with HIPAA regulations and ensure patient confidentiality at all times.
  • Engage in memory care activities for patients with dementia or Alzheimer's to promote cognitive function.
  • Document patient care activities in Electronic Medical Records (EMR) systems accurately.

Qualifications

  • Previous experience in home health care, assisted living, or long-term care settings is preferred.
  • Willing to obtain a Criminal Record check
  • Certification in CPR is required; additional certifications in caregiving or home health are a plus.
  • Experience working with individuals with developmental disabilities care is highly desirable.
  • Strong observational skills for monitoring patient conditions and reporting changes effectively.
  • Ability to work independently while also being part of a collaborative healthcare team.
