About Our Opportunity

We're hiring Care Coordinator Supervisors within our Disability Support Program (DSP), you will play a key role in leading and empowering a team of Care Coordinators. This role is especially key as you support our new community-based framework and the Human Rights Remedy, addressing systemic discrimination against persons with disabilities.

You will provide expert clinical supervision, offering direction and support to ensure effective case management and the highest quality care for our clients. You will mentor staff, troubleshoot challenges, and work collaboratively with external departments and agencies to provide timely and appropriate client services. Leadership and initiative are essential as you oversee the daily operations of your team, making decisions within the framework of legislation, policies, and professional standards.

Primary Accountabilities

As a Care Coordinator Supervisor, you will lead a dedicated team to ensure high-quality care for clients through effective supervision and collaboration. Your responsibilities will include:

Providing clinical and administrative oversight, assigning caseloads, conducting case reviews, and ensuring adherence to policies.Guiding and supporting Care Coordinators and clerical staff, offering direction, advice, and interpretation of policies to help them manage cases effectively.Acting as the primary contact for complex cases, coordinating with other departments and agencies to ensure clients receive appropriate services.Engaging with clients to resolve concerns and clarify care plans.Leading staff development through coaching, evaluations, and creating action plans for ongoing improvement.Managing day-to-day office operations, addressing issues, and ensuring a safe, productive environment.Facilitating team meetings to coordinate services and communicate updates.Collaborating with elected officials, advocacy groups, and community partners to respond to client needs and enhance services. Qualifications and Experience

You are a licensed professional in Nursing (RN or LPN), Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, with at least six (6) years of related experience. You must be currently registered, or eligible for registration, with your professional association.

You are a proven leader with strong supervisory skills, capable of mentoring and guiding a team while making informed decisions in high-pressure situations. Your ability to manage complex cases, provide direction, and foster a collaborative, solution-oriented work environment sets you apart. You bring well-developed critical thinking, problem-solving, and time management skills, which are crucial in overseeing case management and delivering quality services.

You have extensive experience in conducting assessments, interviews, and negotiations, particularly in complex client situations. Your sound judgment, paired with strong communication and interpersonal skills, enables you to navigate challenging circumstances effectively.

Your solid knowledge of child development, as well as the unique needs of children, adults, and families with mental, intellectual, or physical disabilities, ensures you can provide appropriate, person-centred support. You are familiar with the broader health and social services systems, long-term care, case management principles, and risk management, and you understand community development approaches and the coordination of specialized services. Knowledge of relevant legislation, including Homes for Special Care, will be an asset in this role.

A valid driver's license and the ability to travel to various work sites are essential.

We will assess the above qualifications and competencies using one or more of the following tools: written examination, standardized tests, oral presentations, interview(s), and reference checks.

Equivalency

An equivalent combination of training, education and experience will be considered such as:

a Master's degree in either Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies plus four (4) years of related experience

Working Conditions

Most of your work is in a central office environment with frequently required travel to various locations across the province, including government offices, DSP program sites, and healthcare facilities. Pay Grade:  PR 16  

Salary Range:  $2, $3,507.08 Bi-Weekly
