The Social Worker will work as part of the CBI Health team of therapists based out of the Home Health Services Hamilton regional office. The caseload is currently part-time to full-time and will focus on adult patients with a variety of physical and psycho-social conditions. Positions available in the
Hamilton and Niagara
community. This position is ideal for a therapist looking to develop or expand their clinical practice.

What You Need To Be Successful

• You have a Bachelor of Social Work or a Master of Social Work
• You are a registered member in good standing with the Ontario College of Social Workers and Social Service Workers
• 1 to 2 years of community experience is an asset
• You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• You are proficient in the use of Microsoft Word and Excel
• You have ability to travel within the Hamilton and/or Niagara community as part of the CBI Health Home Health Services team
