CasaHealth / HealthLink Outreach works as multidisciplinary health care team focused on providing home care and medication management in the comfort of the patient's home

The Role:

LPNs support clients and their families to achieve independence while maintaining optimal health. You will be responsible for visiting patients in their own homes to provide personal care in a one-on-one setting.

Key Responsibilities:

  • Picking up and dropping off medication(s)
  • Providing nursing supplies
  • Assisting with medication intake and administering medication (including, but not limited to: oral, topical {creams, patches, drops}, subcutaneous, intramuscular)
  • Witnessing medication intake
  • Monitoring all necessary vitals
  • Documenting all medical interventions
  • Formulating, adjusting and following up with care plans
  • Acknowledging and recognizing patient concerns and providing appropriate follow up
  • Communication with team via assigned format (electronic, etc.)
  • Various shifts/routes, as assigned/scheduled

Requirements:

  • Good Standing With BCCNM
  • Valid BC Class 5 Or 7 Driver's License
  • Reliable Vehicle
  • Business Class Insurance
  • First Aid, WHIMS and CPR Certifications
  • Up To Date Immunizations

Job Types: Full-time, Part-time, Permanent, Casual

Pay: $29.50-$32.55 per hour
