We're hiring a Community Health Representative (CHR) to lead community outreach and partnership development in Burnaby and New Westminster. This is a community-based role, not a remote or call-centre position. You'll focus on building and maintaining mutually beneficial referral partnerships with healthcare professionals, community organizations, and local businesses. This means connecting with others who serve similar client groups—helping them learn about our services, while also referring clients their way when appropriate.

You'll also meet with prospective clients to discuss care needs, complete in-home assessments, and support a smooth onboarding experience.

Key Responsibilities:

  • Develop and maintain strong, two-way referral partnerships (clinics, care homes, social workers, pharmacies, etc.)
  • Represent Just Like Family at community events, health fairs, and networking meetings
  • Conduct in-home assessments and help families understand available care options
  • Create personalized care plans and guide clients through onboarding
  • Communicate openly with internal team members to ensure seamless service delivery
  • Maintain accurate records in our CRM ) and track outreach/referral activity daily.
  • Meet weekly and monthly goals related to outreach, referrals, and assessments

What We're Looking For:

  • A friendly, reliable, and self-motivated individual who values honesty and clear communication
  • Experience in face-to-face sales, business development, or community outreach (healthcare or senior services an asset)
  • Strong interpersonal, communication, and organization skills
  • Professional, ethical approach when representing services in the community
  • Comfortable using technology and handling light administrative tasks
  • Valid driver's license and reliable vehicle (road-based role)

Hours:

Full-time, 40 hours per week (permanent)

Monday to Friday

8-hour shifts
