THE ROLE IN A NUTSHELL
- Providing consultation, health teaching and referral to support improved client outcomes within our programs.
- Providing virtual nursing assessments through the Collaborative Care model; these include nursing recommendations and interventions.
- Assisting with the program development, implementation and delivery of Health Services. This includes assess and refer, health teaching, policy implementation recruitment of practitioners, and client follow up.
- Supporting the Nursing team as the program expands/evolves, assisting with the creation of process flows, forms, and service standards.
- Assisting with specialized training for the practitioners providing care within these programs and supporting those clinicians in the expectations surrounding treatment and reporting.
- Being responsible for scheduling assessment/treatment sessions, documenting all interactions with all stakeholders, and providing appropriate reporting updates as required by Inkblot service standards.
- Monitoring compliance, progress, attendance and engagement of all clients engaged in the treatment programs.
- Reviewing all reports received and providing timely reporting to the referring source.
- Development of training materials, patient resources and webinars.
WHO WE'RE LOOKING FOR
Qualifications
- Must be RN (Registered Nurse), Registered Practical Nurse (RPN), or Licensed Practical Nurse (LPN)
- Minimum 3 years of experience
- Registered and in good standing with Provincial College
Skills & Experience
- Bilingual (English/French)
- Critical thinking and ability to make sound decisions in a fast-paced, dynamic setting
- Strong interpersonal, relationship-building, and communication skills
- Experience working in diverse clinical settings including community, hospital and clinics.
- Ability to work collaboratively with different team members and stakeholders
- Strong organizational skills and attention to detail related to quality of service delivery
- High level of proficiency with various computer programs including; Google Drive, Microsoft Office.
- Ability to travel occasionally.
