What We Do There has never been a more exciting time to be at WAHA. We are an integrated health system providing the full continuum of care to the six communities of the western James Bay coast with over 500 employees, physicians, and partners in care. Who You Are
  • Strong understanding of pediatric healthcare and cultural competency in First Nations health issues
  • Proficient in care coordination and case management principles
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in multidisciplinary team environment
  • Strong organizational and time management skills
  • Willingness to become familiar and use Meditech Expanse

What You'll Do
  • Conduct comprehensive assessments of pediatric patients and their families
  • Develop and implement individualized care plans in collaboration with healthcare teams
  • Coordinate referrals to specialists, therapists and community resources and monitor follow up reports
  • Provide education on nutrition, preventive care and chronic disease management
  • Facilitate support groups and community health initiatives
  • Document patient interactions and care plans in electronic health records

What Your Bring
  • Bachelor of Science in Nursing
  • Registered with College of Nurses of Ontario and in good standing
  • Certification in PALS and AED is an asset
  • Respect for diverse cultures and traditions
  • Valid Drivers License – Class G
  • Ability to communicate in the local native Cree language is an asset
