Position Summary
Reporting to the Seniors Services Manager, the Peer Worker will be working for the - community-based health initiative designed to improve access to healthcare for seniors facing barriers to care. Running from November 24, 2025 (date subject to ethics approval) through January 30, 2026, the clinic will operate weekly at 252 Sackville Street in Regent Park.
Each week, seniors in the building will attend the to complete a Health Risk Assessment or drop in for follow-up care and support provided by a Research Nurse and two Peer Workers. The primary goal of this initiative is to help seniors remain healthy and independent by strengthening their connection to primary care, community resources and health education.
GENERAL ACCOUNTABILITIES
The Peer Workers will play a key role in supporting senior clients of the . Working under the guidance of the Seniors Services Manager, Dixon Hall, and Research Nurse on-site, the peer workers will assist clients during by encouraging clients to follow care plans and health recommendations, accompany clients to appointments when required, offer assistance in navigating the healthcare system and access appropriate community services and promote health education and support aging in place.

Duties and Responsibilities
- Review client assessment summaries after health risk assessment is completed
- Support clients in understanding and following care recommendations
- Assist with booking medical or community appointments
- Accompany clients to appointments as needed
- Connect clients to relevant community programs
- Provide health promotion and educational materials
- Conduct client follow-up calls to check in on client progress in achieving goals
- Document client interactions
Qualifications
- Community members living within the Downtown East neighborhoods
- Familiar with Downtown East Toronto and the resources available within
- Good understanding of the systemic and health access barriers in Toronto, for older adults
- Ability to communicate complex information in a clear and compassionate manner
- Ability to listen to clients and help them become comfortable to share their challenges
- Willingness to follow all Dixon Hall workplace policies and procedures
- Comfortable working both independently and as part of a team
- Can follow through on commitments
- Strong verbal and written English language skills; fluency in other languages is an asset especially Cantonese, Mandarin or Vietnamese
- Comfortable using technology for communication and coordination
- Must consent to and successfully pass a vulnerable sector police check, which will be arranged and paid for by Dixon Hall
- MUST be legally eligible to work in Canada
Working Conditions
- Work environment is outside of an office in the neighbourhood; community-based.
- Required to sometimes walk throughout the community, or transit, to accompany clients to appointments or programs.
- The will be running from November 24, 2025 – January 30, 2026 (with a break on December 26 and January 2).
- Peer Workers will be focused on training and preparing in early November 2025 for the to launch in late November 2025.
