POSITION: Urgent Response Case Manager

LOCATION: Barrie, with responsibility throughout Simcoe County

This is a unionized position.

PURPOSE OF POSITION

  • To work in collaboration with the Crisis Response Network Coordinator to support individuals who have been referred through the Urgent Response Process in compliance with the Ministry of Community and Social Services UR Guidelines through the provision of case management throughout Simcoe County
  • To provide limited, short term case management services to individuals through developing and implementing goals and mitigating risks to achieve successful outcomes for the individual
  • To work as a member of a multi-disciplinary team to support individuals in various settings, including their home and in community

STATEMENT OF MAJOR RESPONSIBILITIES

1. Service Delivery

  • Define professional role, boundaries and function of Urgent Response Case Management with the client and/or family/caregiver
  • Provide case management services and supports on a short-term basis
  • Assess individual support needs and risk factors, including health and safety
  • Monitor, evaluate and document individual's progress while receiving services
  • Collaborate with other service providers and/or other sectors as necessary to identify appropriate resources as required
  • Assist clients in coordination and navigation of developmental services sector resources, including resources available through Developmental Services Ontario as well as local resources through Service Coordination Committee
  • Advocate and make appropriate referrals, with consent, for supports and services as required
  • Complete funding requests for clients and manage approved funds, including reviewing invoices, tracking, etc.
  • Work in collaboration with the Service Coordination/Urgent Response Committee, providing information and updates

2. Organization/Teamship

  • To record and report client information as necessary for effective service delivery and case management and prepare progress notes
  • To collect and report service delivery data as required. Computer/keyboarding skills are required
  • To adhere to all agency standards, policies and procedures
  • To regularly attend and participate in supervision and engage in annual performance appraisals
  • To participate in staff training as recommended by the manager and demonstrate initiative in learning and developing new techniques of service delivery
  • To practice honest and effective communication with co-workers
  • Must be aware of and adhere to Health & Safety Policy Rules & Regulations

QUALIFICATIONS:

Education and Experience Level

  • Post-secondary degree at a university level in one of the social or behavioural disciplines with directly related experience (one to two years) OR a community college diploma in one of the social or behavioural disciplines and directly related experience (two to three years) working with individuals with developmental challenges in a community services environment
  • Experience working with persons with developmental delays and mental health issues

Knowledge and Skills

  • Ability to demonstrate sound clinical judgement with respect to complex client situations
  • Excellent communication and negotiating skills
  • Knowledge of community resources for individuals with special needs, proven ability to work effectively on committees, seek out and develop partnership opportunities and formalize service agreements, and relevant legislation
  • Excellent interpersonal and organizational skills, demonstrated ability to manage time and multiple tasks
  • Knowledge of technological applications
  • A working understanding of French Language is beneficial

Valid driver's license and access to a reliable vehicle
